We are now creating a waiting list for new NHS patients wishing to be seen by our Dental Therapist.

If you would like to be added to the waiting list, you must attend the practice in person to complete a waiting list form and provide your Health & Care Number.

Please note:

• Completing this form does NOT mean you are registered with the practice at this time.
• Due to the high demand for NHS dental appointments, we currently have a long waiting list for appointments with a dentist.
• At present, we are only able to offer certain treatments with our Dental Therapist.

We appreciate your patience and understanding while we work through the demand for NHS care.
