Final shot of a soft tissue graft for Pontic site augmentation.

4 months prior we placed 1 immediate implant with some bone grafting and most importantly a custom healing abutment. That set the tissues up perfectly for Pontic site augmentation and achieving a natural looking bridge.

This was a de-epithelialised FGG harvest placed in a tunnel buccally with a palatal peninsula release flap and papillae augmentation. An insanely effective technique by the masters Huerzeler and Zuhr. I would 100% recommend their soft tissue masterclass.

Materials - custom Atlantis abutments with a zirconia bridge. It’s cruical the fit surface of the bridge copied the tissue conturing perfectly for the UR1 but recreated the ideal contour for the UR2 pontic. So important that it had a hyperpolished, non glazed fit surface.

What do you do with a low sinus floor when the patient can’t have a sinus graft?

A little workaround with an immediate implant UR4, a bit of expansion UR5 and crucially a distally angled implant and multi unit abutment UR7. A combo of a custom healing abutment, a stock abutment and an MUA heal cap set the tissues up perfectly to achieve a natural looking bridge.

This case involved full arch rehabilitation to restore function, aesthetics, and confidence with immediate fixed teeth. LL2 was removed at the review stage and replaced to ensure the best long term outcome for the patient.
