Big defect on palatal side of upper anteriors  and  moderate wear on the other sectors

Attrition & Erosion
Etiology : Bruxism and GERD

Patient complaints :  replace 16 / maxillary and mandibular crowding  / Aware of tooth wear

In my opinion,  increasing VDO with posterior table tops would be overtreatment

 

Treatment plan :

-   Orthodontics  (with avulsion  of 41 – after studying occlusion and perio factors )

-   Direct composite for exposed dentine on posteriors

-   Sinus Lift + implant 16

-   Anterior guidance, wear correction and smile make over by direct composites on 13-12-22-23  /  palatal veneers no prep  (cerasmart and layering composites on 11-21

Please teach your patients that proportions first 🙏 I often see black triangles closed in the expense of esthetics forms and proportions ...

Black triangles Closure after periodontal therapy.
Here to completely closed the black triangle 12/11 2 options:
- composite 12 mesial and an unesthetic form
- composite 12 mesial and 11 distal and loose the symmetry of central incisors

I chose the third option : not close it completely ! For a natural esthetic smile 😁
