** PEG LATERAL **

A 21-year-old patient consult for an aesthetic complaint regarding tooth 12 with an old composite restoration.

🎯 The goal is to meet her aesthetic expectations while using a purely additive approach. Let's go for Layering composite.

Analyzing the morphology and color mapping of the contralateral tooth (22) are key steps.

** Global Rehab and Wear **

A very kind patient seeking to improve the aesthetics of her smile. She is aware of the wear on her teeth and the associated functional issues.



Treatment plan:


- Etiology corrections: erosion (history of high lemon juice consumption) and attrition: bruxism (Hartmann-Bratslavski technique) and inter-arch relationship/deep overbite.

- Treatment planning with Smilecloud

- Increase VDO

- New mandibular position (aiming to improve the anterior relationship - Orthodontic was declined).

- Implants at sites 16 and 13 (extensive decay under the crown on tooth 13).

- Extraction of 18.

- Ceramic restorations on the maxillary arch.

- Composite restorations on the mandibular arch for financial reasons (injected technique for posteriors / layered technique for anteriors) after teeth whithening.

- Night guard.



Difficulties:

- Reduce overbite only with prosthetic work and no orthodontic (compromise).


- The old implants are Straumann Bone Level implants with very limited soft tissue height, and tissue level are now juxta- or supra-gingival. Removing the implants would have been a complex solution involving significant tissue loss. We therefore opted for a cement-retained prosthesis using custom abutments to simplify the treatment. Only 13 is screw-retained.

- Achieve a natural look despite a request for a very white color by adding plenty of texture

- Requires very regular monitoring and, undoubtedly, future replacement of the mandibular composites.

** LOCALIZED TOOTH WEAR **

 

Big defect on palatal side of upper anteriors  and  moderate wear on the other sectors

Attrition & Erosion
Etiology : Bruxism and GERD

Patient complaints :  replace 16 / maxillary and mandibular crowding  / Aware of tooth wear

In my opinion,  increasing VDO with posterior table tops would be overtreatment

 

Treatment plan :

-   Orthodontics  (with avulsion  of 41 – after studying occlusion and perio factors )

-   Direct composite for exposed dentine on posteriors

-   Sinus Lift + implant 16

-   Anterior guidance, wear correction and smile make over by direct composites on 13-12-22-23  /  palatal veneers no prep  (cerasmart and layering composites on 11-21
