Finally our Master piece article on QDT 2021/2022, a different approach whenever possible to take into consideration on daily planning. When the transition line between the alveolar ridge and the prosthetic restoration is revealed during a smile, the typical treatment is to remove bone vertically to hide this transition below the smile line. However, this procedure considerably alters the lip support, which some patients find greatly displeasing and often describe as “losing part of their identity.” In our experience, many patients are re- luctant to consider implant treatment because they are afraid of losing their original smile and, with that, their iden- tity and personality.
This article suggests a modification of the traditional protocol that may be applied in select patient situations to avoid continuous bone loss and exposure of the prosthe- sis–alveolar bone transition line during a smile, improving esthetics with the All-on-4 concept.

Clinical decision-making, recession on dental implants, plastic surgery.
Finally our article has been published in the journal of periodontology this month !with the invaluable help and talent of Dr. Gustavo Ávila ! In this issue we present a new classification system for peri-implant marginal mucosa defects PMMD, as well as clinical recommendations pertaining to the prosthetic and surgical management of each type of PMMD, illustrated with practical examples in order to facilitate decision-making processes in daily practice. A diagnosis of the implant restorability is taken into account before planning any surgical plan as never before.

Lower anterior case series/simple option, a crown substitution #42.
Challenge here was to solve the dark abutment color and create a translucent, high value crown due to the previous bleaching on a heavy smoker, we waited 6 months after bleaching for stability of the value !

Lower anterior case series/simple option !
Never easy to solve lower anterior dilemas specially when big defects is present on a single tooth !
Challenge here was to solve the vertical and horizontal bone loss
from infected lateral incisor #41.
84 years old female patient presented to me with this situation and her only complaint was no implant please because I won’t live longer ! I want a simple solution that will stay there for a little while !
The plan was to perform a Maryland bridge single wing from the adjacent stronger tooth either central or lateral. Day of tooth extraction perform GBR then wait 5 months to perform the crestal ridge lap with tuberosity CTG to compensate the bone collapse using a tunnel approach through a vista incision ! 3 months later delivery of the final Maryland zirconia bridge. During all phases of the treatment original tooth was kept and bonded to the adjacent teeth as a provisional.

Lower anterior case series !
Never easy to solve lower anterior dilemas specially when implants are present, here, a 4 unit bridge on 2 steri-oss implants that I placed 20 years ago !
At that time not much into grafting bone and tissues as today so we planned for a PFM pink ceramics instead great job performed by ceramist Mr. Iñigo Casares.
Challenge here was to solve the vertical and horizontal bone loss
From infected canine #43 !
Sequencing is crucial in order to lose the minimum vertical bone height possible!
The plan was to perform an implant on #43 and level the interproximal tissues to blend with pink tissue of the adjacent bridge as well as providing the necessary buccal volume to mimic contralateral
canine #33.
One stage surgery was executed
extracting #43 with immediate implant placement. The bridge was unscrewed to perform the most ideal conditions for the GBR, xenograft was used and tCTG as a membrane ridge lap on the slim healing abutment area stabilized bucco lingualy.
After 3months of healing
Final impressions was taken and a screw retain ASC restoration Procera was fabricated !

Lower anterior case series !
Never easy to solve lower anterior dilemas specially when an implant is present and teeth around are infected and need to be extracted. Thanks God the implant was in good health which it was a less problem to worry about. Sequencing is crucial in order to lose the minimum vertical bone height possible!
The plan was to perform a bridge from the existing implant #41 to a new implant on #32.
An alternative implant treatment sequential approach was considered :
1• first extracting #31 with GBR, xenograft and using tCTG as a membrane ridge lap on the extracted tooth area, as well as grafting adjacent implant #41 and #32 on the buccal aspect. Provisional was provided same day than surgery splinting implant #41 to tooth #32.
2• 3 months later immediate implant placement NobelActive 3,5x13mm. was inserted with a circular tCTG using a slim temporary abutment and Provisional restoration adapted to the slim prosthetic design.
Another 3months later of healing
Final impressions were taken and titanium abutment deliver on #32 and a cemented Procera bridge !

Lower anterior case series ! Single lower lateral implant incisor with horizontal and vertical defect which is never easy !
Implant surgery was performed on a one stage approach with a NobelActive 3.0x13mm. on #32 with 7mm. height slim healing abutment + crestal CTG donor site : tuberosity. After 3 months healing, provisional restoration was fabricated to mold soft tissues and left for another 3 months ! Mild gingivectomy was performed on the anterior area to level gingival levels unfortunately CEJ was exposed on #41 which was not a good choice ! A feldespathic veneer was performed on #31 in order to blend the left side restorations to the contra lateral natural teeth.
Then final restoration here was a titanium abutment due to the low value restoration on the NobelActive 3.0 which is always a pity not having an ASC abutment in zirconia for this implant ! On top Procera zr crown.
Class 3 malocclusion from #14 to 22 direct composite veneers partial or complete, then posterior implant bridges upper left and lower right were in charged of the vertical dimension raise in order to level the occlusal planes and provide the best anterior guidance and coupling!

Lower anterior case series ! Single lower central implant incisor with horizontal and vertical defect which is never easy ! Patient interested only on solving his central incisor loss.
Implant surgery was suggested and performed on a one stage approach with a NobelActive 3.0x13mm. on #31 with 7mm. height slim healing abutment + crestal CTG donor site : tuberosity. After 3 months healing, we can appreciate the volume achieved, then provisional restoration was fabricated to mold soft tissues and left another 3 months ! Then final restoration here we decided with Yoshi my lab technician to go with a titanium abutment due to the low value restoration on the NobelActive 3.0 which is always a pity not having an ASC abutment in zirconia for this implant ! On top Procera zr crown.

Lower anterior case series ! 10 years post treatment of a lower anterior delayed implant case that I did, which goes against with what I usually perform but here it came out nice even though I could have improved it if I would have done a slim healing abutment instead. Lower delayed implant placement represents a challenge in many aspects; implant position as adjacent roots are typically convergent to the midline, small medio-distal and bucco-lingual space besides a dense bone with huge buccal concavity, and the smallest tooth to restore in the entire mouth, and small place to graft crestally, a challenge! Performing a one abutment one time concept represents a complex treatment for many dentists besides the fact that increases the difficulty when grafting on the crest which is the best location to improve the papillae. Placing the abutment from day one is a bad idea in that sense as it takes to much of space crestally minimizing space for the crestal CTG. One example is this case explained on my book evolution co-author with Dr. Markus Blatz in which I detailed all steps. If you precisely analyze the result you can see the difference in papilla heights which could have been improved regardless of the close proximity of the implant !

Lower anterior case series ! Lower Bridges represents an additional difficulty, as free gingival level topography is higher in both central incisor respectively than lateral incisors and creating this on a vertical and horizontal ridge defect represents a big challenge. Additionally lower anterior is an area where the anatomy is tricky in order to get vertical flap mobility thanks god both flaps can be mobilized in this area! In this case I chose to fix the problem with a 4 unit bridge and doing CTG from both tuberosities as patient could provide me with big amount of dense tissue. 2 rounds of tuberosity CTG were performed within 3 months of healing in between them. Then with the provisional restoration the Pontic area were managed !
