Reendodoncia, descompresión maxilar tras agudización a los 6 meses, microcirugía periapical con acceso vestibular y palatino (defecto through and through), regeneración ósea y con membrana en vestibular y palatino. Control a los 3 meses todo parece ok.

Re-treatment, maxilar decompression after accute pain at 6 months after retreatment, periapical microsurgery with buccal and palatal access (through and through defect), bone and membrane regeneration in the buccal and palatal site. Check up at 3 months seems ok.

Reendodoncia de 3.6. Todos los pasos en el tratamiento de endodoncia son importantes, pero sobre todo hay que hacer hincapié en la irrigación como hicimos en este caso. La paciente vino con flemón que desapareció en la segunda cita. En estos casos activamos el irrigante desde la primera visita ya que queremos que en la segunda visita el diente se encuentre en condiciones óptimas para obturar obteniendo los mejores resultados posibles a largo plazo. Los conductos laterales que aparecen obturados en la radiografía final condicionan el éxito del caso. / Retreatment in 3.6. Every step in endodontics matter, but it is imperative to make special effort in activating irrigants like we did in this case. The patient came with a big abcess that dissapeared in the second visit. In these cases we activate the irrigants since the first visit because we want to have the best conditions to be able to finish having the best results for the long term. Lateral canals that appear obturated in the final x-ray determine the success in this case. 🔬

Tratamiento en paciente infantil. Esta paciente es una paciente muy especial porque su madre la trajo desde lejos con mucho esfuerzo en tren para salvar la muela de su hija. Estos pacientes responden muy bien al tratamiento químico mecánico. Este caso lo realizamos en dos citas e hicimos apicoformación con MTA en la raíz palatina debido al gran calibre de esta raíz. / Treatment in a young patient. This is a very especial patient because her mother brought her making a big effort coming from far by train to save her daugther’s molar. These patients respond very well to the Chemical-mechanical treatment. We did it in two sessions and made an apexification with MTA in the palatal root because it had a wide caliper.

Cuando trabajas con una periodoncista que cuida de los dientes de sus pacientes y te pide que le salves una muela a uno de sus pacientes…. En este caso no pudimos reconstruir la pared mesial previamente ya que el dentista que accedió antes que nosotros había colocado el algodón debajo de la encía en lugar de la cámara pulpar, y la encía no estaba en condiciones para poder trabajar. Realizamos el tratamiento de conductos y dejamos la reconstrucción para la siguiente cita. En estos casos siempre utilizo composite consistencia esmalte para disminuir la retención de placa./When you work with a periodontist that cares a lot about teeth and asks you to save one of her patient’s molar…. In this case we could not reconstruct the mesial wall before the nerve treatment because the previous dentist had put cotton underneath the gingiva instead of putting it inside the pulp chamber, and the gingiva was not in good conditions to work with it. We finished the nerve treatment and left the reconstruction for the next appointment. In these case I always use composite with enamel consistency. 🔬 💡

La mejor endodoncia es la que no se hace. En este caso hicimos un Recubrimiento Pulpar Indirecto con MTA NeoPutty (Avalon). Antes de realizar cualquier recubrimiento pulpar SIEMPRE levantamos pared antes de colocar el material de recubrimiento para asegurarnos de que los márgenes serán material restaurador y así aseguramos un buen sellado. Después desinfectamos, colocamos el MTA, y terminamos la restauración. Espero que todos sigamos teniendo un buen endo-día/ The best endo treatment is the one that is avoided. In this case we did an indirect pulp capping with MTA NeoPutty (Avalon). When we do a pulp capping, we ALWAYS reconstruct all dental walls before we put the MTA to make sure that the material at the edge is composite, and that everything will be well sealed. Then we desinfect, put the MTA and finish the filling. Hope you all keep having a good endo day.

Tormenta eléctrica ayer en Denia. No nos dejó dormir pero hemos venido a hacer endodoncias igualmente / Electricstorm in Denia yesterday. It didn’t let us sleep but here were are doing endos.

3.5 en el que encontramos dos conductos pero al obturar observamos bifurcación apical del conducto mesiolingual. El haber realizado un buen protocolo de irrigación final permitió desinfectar y obturar este nuevo conducto con cemento, pero ¿será estable a largo plazo? De momento llevamos 6 meses y parece que la lesión ha curado 🤞/ Tooth 20 in which we found only two canals but after obturation we could see an apical branch of the mesiolingual canal. A good irrigation protocol allowed us to desinfect and obturate with cement the new canal, but is this stable in the long run? Its been 6 months for now and it seems the lesion has healed 🤞
