This tooth had been restored twice before,
yet the patient kept complaining of food accumulation and pain.

After multiple preparations, posterior teeth often tend to lose vitality —
but in this case, the molar was still vital, so preserving it was the priority.

A direct overlay using the Bioclear technique was selected instead of an indirect option.
To solve the main complaint, each proximal contact was restored separately,
allowing better separation and more predictable contact anatomy.

Simple changes in approach can completely change the outcome.

Lower right quadrant restored with two immediate implants in the second premolar and second molar ,
For the premolar site, I kept part of the root at the beginning to guide my drilling and keep the direction stable.
After the osteotomy was completed, the remaining part was removed and the implant was placed .
Both implants were finished with custom healing abutments to shape the soft tissue from the start.
The CBCT planning and the final positions matched very closely.

Immediate vs Delayed Implant — Same Area, Different Bone

These photos show how bone can look completely different depending simply on when the implant is placed.

-Immediate implant
When the implant is placed right after extraction, the socket is still intact.
The bone width is good, the walls are clear, and the drilling path is naturally guided.
Everything feels more controlled because the anatomy is still “fresh.”

-Delayed implant
In the site that was extracted a while ago, the bone has already remodeled.
You can see natural resorption — the ridge becomes narrower, and the original socket shape disappears.
That’s why the delayed side looks thinner and has a clear defect compared to the fresh socket next to it.

Bottom line:
Same patient, same area… the only difference is timing.
Bone changes a lot when the site heals without an implant.

Sometimes the sinus bone looks acceptable,
but not quite enough for a stable implant in the long run.

A small closed sinus lift was done to gain a bit more height
and give the implant better support.
The membrane lifted smoothly and the implant had solid primary stability.

The second premolar needed more biologic width,
so crown lengthening was done to prepare a healthy base for the final restoration.
