I didn’t know about Enamel Renal Syndrome until I met this patient.

ودي من أكتر الحاجات اللي بحبها في الـclinical practice…
إن أحيانًا case واحدة ممكن تفتح لك باب كامل للتعلم.

المريض بدأ معانا وهو عنده 15 سنة، والـclinical picture في البداية كان يشبه Amelogenesis Imperfecta: severe enamel loss, multiple missing teeth, compromised posterior support وloss of VDO.

لكن أثناء الـinvestigations والـsurgical planning، اكتشفنا إن الـenamel changes كانت جزءًا من صورة أكبر:
Enamel Renal Syndrome.

ومن هنا الـcase اتحولت من مجرد prosthetic rehabilitation إلى 3-year multidisciplinary journey مليانة diagnostic, surgical & prosthetic challenges.

في الـcase documentation ده هشارك معاكم:
الـdiagnosis، treatment planning، إزاي تعاملنا مع الـloss of VDO، وأهم الـprosthetic challenges والقرارات اللي أخدناها في كل مرحلة.

والـcase لسه ongoing — بعد اكتمال implant healing، هنستبدل الـlower removable prosthesis ونكمل للـdefinitive implant-supported rehabilitation.

Sometimes the best cases don’t just test what we know…

A little clinical tip that might save your veneers… 🤍🦷

خصوصًا لو بتتعامل مع patients مغتربين، بييجوا في الإجازة يعملوا جزء من الـtreatment ويسافروا قبل ما نخلص الـfull rehabilitation.

Veneers are done… implants are still healing… and the patient has to travel. ✈️
So what happens to the bite in between?

في حالات loss of posterior support، ترك الـpatient فترة طويلة بدون تعويض مناسب ممكن يزود الـ
tendency toward an edge-to-edge anterior bite➡️Shear Stresses on Incisal edges⚡️⚡️

ولو إحنا بالفعل عاملين anterior veneers، الـshear stresses عليها هتكسرها او علي الاقل تفكها🌚.

هنا واحد من الحلول المؤقتة اللي بحب أستخدمها:

An Essix appliance with posterior teeth replacement.

بيشتغل كـ
interim restoration during osseointegration period
لحد ما المريض يرجع ونقدر نكمل الـdefinitive implant restorations.

The goal here is to protect the anterior rehabilitation while treatment is still in progress. 🦷✨

Sometimes, the temporary phase is just as important as the final restoration.

حبيت أشارك معاكم case من أكتر الحالات اللي كانت challenging بالنسبة لي، سواء في الـdiagnosis أو الـtreatment planning.

الـclinical presentation في البداية كان واضح، لكن مع الـfull documentation والـocclusal analysis، خصوصًا مقارنة MIP vs Centric, smile design… كمان ضيق الوقت لان الpt مسافر…فكرنا بطريقه مختلفه.

حاولت أوثّق في الـslides الـdecision-making process بالكامل:
Diagnosis → Occlusal analysis → Treatment planning → Ex*****on → Final outcome → Limitations.

الهدف من مشاركة الحالة مش تقديم approach باعتباره الحل الوحيد أو المثالي لكل الحالات المشابهة، لكن مشاركة إزاي اتخذنا القرارات في الحالة دي تحديدًا، بناءً على الـavailable records والـpatient-related factors والـclinical limitations.

والـcase كانت multidisciplinary teamwork 🤍

One of the most common questions I received after sharing my rehabilitation cases was👇👇👇

"What was the first step in planning this rehabilitation?"

بالنسبالي، أول خطوة مش اختيار اللون...
ولا اختيار الmaterial ...
ولا حتى تصميم الابتسامة.

أول حاجة بدور عليها هي:

Are the maxillary central incisors in the correct position and length?

لأن الـ central incisors هم المرجع الأساسي لباقي عناصر الابتسامة‼️.

If the central incisors are too short, everything else becomes difficult to design correctly.

وبالنسبالي، لما ألاقي طول الـ central incisors أقل من 10 mm، ببدأ أسأل نفسي:

❓ هل محتاجين يطولوا؟
❓ ولو محتاجين يطولوا... الطول ده ييجي منين؟

والإجابة غالبًا بتكون موجودة في صورتين بسيطتين جدًا💡💡.

ودي مش مجرد صورة والمريض بيبتسم فيها وخلاص.

I usually ask the patient to say:

"Cheese, فسيخ, بطيخ… I don’t really care what the patient says, as long as I capture the maximum smile.” 😄

في الصورة دي بنقيّم كمية الـ gingival display أثناء الابتسامة الكاملة📏📏.

Ideally, gingival display should not exceed 2 mm.

لو ظهر أكتر من 2 mm من اللثة، فغالبًا عندنا high smile line (gummy smile).

وده معناه إن جزء من مشكلة قِصر الأسنان ممكن يتحل gingivally عن طريق crown lengthening أو gingivectomy، فنبقي طولنا central incisors وكمان عالجنا مشكله gummy smile✅✅.

ودي أهم صورة في الموضوع كله.

المريض مش بيقفل بقه للكاميرا وخلاص.

I usually ask the patient to say "Emma" ثم يسترخي، أو ياخد عدة أنفاس عميقة قبل التصوير.

في الصورة دي المفروض نشوف تقريبًا 2–4 mm من الـ maxillary central incisors.

لو الـ incisal display أقل من كده، فغالبًا محتاجين نفكر في زيادة الطول incisally وده يعني ازود VDO (يعني لازم الmolars يدخلوا معانا في الdesign🫩🫩😉).
وده الي بنسميه👇👇👇👇

✨✨The 2–2–4 Rule

▪️ Gingival display in maximum smile ≤ 2 mm

▪️ Incisal display in repose photo = 2–4 mm

صورتين فقط...

لكنهم بيجاوبوا على سؤال مهم جدًا:

Where should the additional tooth length come from?

في الحالة دي كان عندنا indication للاتجاهين.

الفيديو أظهر high lip line، وبالتالي كان فيه إمكانية لزيادة الطول gingivally.

لكن الpt ماكانتش حابه فكره الـ crown lengthening .

So we proceeded with incisal lengthening only, achieving a more youthful smile while respecting the patient's preference 🥰🥰

“The price of chasing esthetics… while ignoring biology.”

الحالة دي كانت reminder مهم جدًا إن محاولة الـ camouflage للـ gummy smile ممكن تتحول biologically لكارثة… لو الـ prosthetic planning تجاهل الـ tissues.

The patient presented with:
▫️ Severe gummy smile (~8 mm)
▫️ Proclined maxillary anteriors
▫️ Chronic gingivitis around old crowns

الـ previous treatment حاول يخفي الـ gummy smile باستخدام excessively deep subgingival crowns.

The problem?
🚨 Biologic width violation.

ودي من أكتر أسباب الـ anterior esthetic failures اللي بتبدأ silently للأسف.

المريضة عاشت سنين مع:
❌ Persistent inflammation
❌ Bleeding gingiva
❌ Poor cleansability
❌ Esthetic dissatisfaction رغم كل الشغل restorative اللي اتعمل

وقبل ما نفكر في esthetics…
أول سؤال لازم نسأله:

“Are the tissues biologically healthy?”

✨ Treatment sequence:
1️⃣ Surgical crown lengthening

2️⃣ Healing & tissue stabilization phase

3️⃣ Prosthetic rehabilitation مع التعامل مع:
▫️ VDO deficiency
▫️ Midline discrepancy
▫️ Limited occlusal clearance
▫️ Arch form correction
▫️ Proclination camouflage
▫️ Emergence profile redesign

Important prosthodontic decisions:
✔️ Separate units instead of splinted crowns
عشان better hygiene & tissue stability

✔️ Inclusion of premolars
لتوسيع الـ arch form من V-shape إلى broader U-shape

✔️ Controlled labial reduction
لتقليل الـ protrusive appearance بدون overbulking restorations

✔️ Careful shade selection
لأن الـ brighter shades كانت هتزود الـ visual proclination

⚠️ Clinical message:
لو جالك failing anterior crowns + chronic gingivitis…
ما تبصش على الـ crowns بس.

Always evaluate:
✔️ Margin depth
✔️ Emergence profile
✔️ Biologic width integrity
✔️ Occlusal clearance
✔️ Functional envelope
✔️ وهل الـ prosthetic camouflage بيحاول يخفي skeletal problem أصلاً؟

Because sometimes…
the restoration isn’t failing mechanically.

It’s failing biologically🤕

❌❌❌ A very similar issue was publicly noticeable in actress Mai Kassab’s previous anterior restorations before correction.”
