Management of an Open Margin Crown 🦷
​When an old crown fails at the margin, it invites a clinical puzzle underneath. This case presented a challenging deep dark discoloration, making us ask the classic diagnostic question: Recurrent Caries or discoloration.
​Here is how we navigated this step by step:
​Isolation & old restorationremoval: Removing the compromised restoration under strict rubber dam isolation to expose the true extent of the lesion.
​Differential Diagnosis: Careful excavation and inspection revealed the true nature of the deep stain using caries detection dye.
​Structural Reinforcement: Achieving a textbook 2 mm ferrule effect for optimal biomechanical resistance and retention.
​Advanced Restoration: Utilizing fiber reinforced composite base build-up (everX) to reinforce the compromised tooth structure prior to final impressions and the precision multilayer zirconia milled final crown.
