Secondary Reconstruction of Panfacial Fractures with 3D-Assisted Surgical Planning
Alhamdulillah, we successfully performed a complex secondary reconstruction for a patient presenting with panfacial fractures. The patient had previously undergone an emergency surgical intervention in his home country under challenging circumstances. Unfortunately, the initial management failed to achieve adequate anatomical reduction and stable fixation, resulting in fracture non-union and significant post-traumatic facial deformity.
Following comprehensive clinical and radiological evaluation, a meticulous surgical plan was established. A patient-specific 3D-printed model was fabricated for preoperative planning, anatomical analysis, and pre-bending of the fixation plates.
Surgical exposure was achieved through a bicoronal approach combined with intraoral vestibular incisions, providing adequate access to the upper, middle, and lower facial skeleton. Open reduction and internal fixation (ORIF) were then performed using appropriately contoured titanium plates and screws to restore the facial skeletal buttresses, achieve anatomical reduction, re-establish facial symmetry, and optimize both functional and aesthetic outcomes.
We thank Allah for His guidance and pray for the patient's uneventful recovery and an excellent long-term functional and aesthetic result.
