Back in 2021, I diagnosed a pt. with necrotic upper 6 showing Periapical Radiolucency. It was asymptomatic, so the pt. neglected to get it treated 😔

Now in 2026, same pt. showed with cellulitis. That upper 6 was decayed, with furcation involvement & large Periapical lesion perforating the sinus 😱 It was hopeless & indicated for extraction 😤

CBCT ☢️ examination showed high risk of oro-antral communication & roots slippage in sinus during extraction 🤨 Atraumatic extraction was performed… but the fracture of apical third of palatal root was unavoidable 🤓 That palatal root apex was then secured with K-file #30 & safely removed 🧐
