removal of coronal dentin triangle decreases the chance of coronal ledges and over-stressing of the instrument , by providing a slightly more straight path to the middle and apical thirds of the canal , over-flaring should be avoided though to preserve pericervical dentin as it is essential for tooth survival

A Challenging Scenario

We recently encountered an intriguing case at our clinic: a patient presented with Upper right 7 SIP just one month after cementation of a newly fabricated 4-unit zirconia bridge. Here are the key highlights:

Preoperative X-ray revealed relative calcification and tilting of the isolated 7, making access a challenge.

The most challenging step? Gaining access through the zirconia bridge.

Periodontal probing of the CEJ (Cemento-Enamel Junction) plays a crucial role in creating a mental image and planning access at the center.

Access must be parallel to the long axis of the tooth itself, especially considering the molar's tilt.

Calcification was managed using C hand files and high-cutting-efficiency gold rotary files following a crown-down protocol.

The procedure was successfully concluded with a modified WVC Obturation.

Another day, another fascinating dental puzzle solved!
