𝗔𝗽𝗶𝗰𝗮𝗹 𝗰𝗼𝗻𝘁𝗿𝗼𝗹 𝗶𝘀 𝗻𝗼𝘁 𝗮 𝗹𝘂𝘅𝘂𝗿𝘆 — 𝗶𝘁 𝗶𝘀 𝗮 𝗽𝗿𝗲𝗿𝗲𝗾𝘂𝗶𝘀𝗶𝘁𝗲 𝗳𝗼𝗿 𝗽𝗿𝗲𝗱𝗶𝗰𝘁𝗮𝗯𝗹𝗲 𝗼𝗯𝘁𝘂𝗿𝗮𝘁𝗶𝗼𝗻.

Young permanent teeth with incomplete apical development often present with a 𝘄𝗶𝗱𝗲, 𝗱𝗶𝘃𝗲𝗿𝗴𝗲𝗻𝘁 𝗮𝗽𝗲𝘅, leaving little or no natural apical constriction to contain obturation materials.

In such cases, placement of an 𝗠𝗧𝗔 𝗮𝗽𝗶𝗰𝗮𝗹 𝗽𝗹𝘂𝗴 can establish an artificial apical barrier, providing:
• 𝗖𝗼𝗻𝘁𝗿𝗼𝗹𝗹𝗲𝗱 apical termination of the obturation�• 𝗥𝗲𝗱𝘂𝗰𝗲𝗱 𝗿𝗶𝘀𝗸 of material extrusion�• 𝗔 𝘀𝘁𝗮𝗯𝗹𝗲 interface for subsequent obturation�• 𝗚𝗿𝗲𝗮𝘁𝗲𝗿 𝗽𝗿𝗲𝗱𝗶𝗰𝘁𝗮𝗯𝗶𝗹𝗶𝘁𝘆 in managing challenging apical anatomy

𝗠𝗮𝗻𝗱𝗶𝗯𝘂𝗹𝗮𝗿 𝗳𝗶𝗿𝘀𝘁 𝗺𝗼𝗹𝗮𝗿 𝗥𝗲𝘁𝗿𝗲𝗮𝘁𝗺𝗲𝗻𝘁 - 𝗟𝗮𝘁𝗲𝗿𝗮𝗹 𝗿𝗼𝗼𝘁 𝗽𝗲𝗿𝗳𝗼𝗿𝗮𝘁𝗶𝗼𝗻 𝗮𝗻𝗱 𝟮 𝘀𝗲𝗽𝗮𝗿𝗮𝘁𝗲𝗱 𝗶𝗻𝘀𝘁𝗿𝘂𝗺𝗲𝗻𝘁𝘀:

𝗖𝗮𝘀𝗲 𝗔𝘀𝘀𝗲𝘀𝘀𝗺𝗲𝗻𝘁 𝗮𝗻𝗱 𝗜𝗻𝗶𝘁𝗶𝗮𝗹 𝗠𝗮𝗻𝗮𝗴𝗲𝗺𝗲𝗻𝘁:
The existing gutta-percha was carefully removed to establish clear access and adequate visualization of the root canal system.
The first objective was the retrieval of the two separated instruments from the mesiobuccal and mesiolingual canals.

Under 𝗺𝗮𝗴𝗻𝗶𝗳𝗶𝗰𝗮𝘁𝗶𝗼𝗻, 𝘂𝗹𝘁𝗿𝗮𝘀𝗼𝗻𝗶𝗰 was used to carefully loosen and retrieve the instrument fragments while preserving as much radicular dentin as possible.

𝗖𝗮𝗻𝗮𝗹 𝗡𝗲𝗴𝗼𝘁𝗶𝗮𝘁𝗶𝗼𝗻 𝗮𝗻𝗱 𝗣𝗲𝗿𝗳𝗼𝗿𝗮𝘁𝗶𝗼𝗻 𝗠𝗮𝗻𝗮𝗴𝗲𝗺𝗲𝗻𝘁:
Following instrument retrieval, attention was directed toward identifying and negotiating the 𝗼𝗿𝗶𝗴𝗶𝗻𝗮𝗹 𝗰𝗮𝗻𝗮𝗹 𝗽𝗮𝘁𝗵𝘄𝗮𝘆 associated with the perforated root.
A manual #𝟭𝟬 𝗞-𝗳𝗶𝗹𝗲 was used with gentle tactile control to negotiate the canal and confirm the original trajectory.

Once the 𝗼𝗿𝗶𝗴𝗶𝗻𝗮𝗹 𝗰𝗮𝗻𝗮𝗹 𝗽𝗮𝘁𝗵𝘄𝗮𝘆 had been established, mechanical preparation was performed using rotary instrumentation with the 𝗧𝗮𝗰𝘁𝗶𝗹𝗲-𝗖𝗼𝗻𝘁𝗿𝗼𝗹𝗹𝗲𝗱 𝗔𝗰𝘁𝗶𝘃𝗮𝘁𝗶𝗼𝗻 (𝗧𝗖𝗔) technique. This allowed controlled canal preparation while maintaining continuous awareness of the existing anatomy and minimizing unnecessary dentin removal.
