• Upper premolar with a large ledge due to previous RCT.
• Canal managed using EndoCem (Maruchi) for ledge repair, MTA apical plug, and backfill obturation using warm Gutta percha.
• Upper first molar presenting a deep apical split in the palatal canal adding significant anatomical complexity.

✨ BROKEN FILE RETRIEVAL IN A C-SHAPED CANAL ✨

This case was referred to our clinic with 3 broken files inside a C-shaped canal 🦷.

🔬 Using the dental operating microscope, ultrasonic tips, and thorough irrigation, we managed to carefully retrieve all separated instruments and complete the retreatment orthogradely.

✅ Obturation was performed with Bioceramic sealer and MTA apical plug in distal canal followed by backfill.
✅ For the restoration, we used:
• Ionoseal for a strong coronal seal
• Ribbond fiber + EverX as dentin replacement, acting as a stress distributor.

This approach was ideal for the deep Class I cavity (with high C-factor), ensuring better bond strength, fracture resistance, and long-term success.

Proud of the teamwork and precision that turned a highly complex case into a successful outcome 💪✨

🦷 First Apical Surgery – Clinical Case 🦷

This case was referred by a colleague for the retrieval of a separated instrument in the lower first premolar.
However, the fragment extended beyond the apex in a curved canal, which made nonsurgical retrieval not feasible.

To preserve the tooth, apical surgery was the best treatment option. The surgery was completed successfully, and the root-end obturation was performed using MTA, ensuring a biocompatible and predictable seal.

I’m truly grateful for the guidance of Dr. and the invaluable support of team in this case.
This marks my first apical surgery, a memorable milestone in my clinical journey 🤍🛣️
