New case🔥💪
Patient arrived to the clinic with a swelling related to the left side of the mandible. And upon examination and x-rays, an incomplete Root Canal Treatment was found in tooth #36 with two broken files, one wedged in the mesio-buccal canal and the second extending beyond the mesial root, and a large periapical radiolucensy related to both roots. Root Canal Retreatment was performed on two visits. The first visit included access cavity preparation and complete caries removal, patency and erigation of the mesio-lingual and the two distal canals , slight troughing around the broken file using an ultrasonic endo tip ( ED4) which exposed multiple perforations in the coronal third of the canal, and application of calcium hydroxide intracanal medication paste. The second visit was performed after one week and included troughing around the broken file followed by an inner wall delodging of the file causing the file to be extuded from the canal, rotary shaping and cleaning using irrigation protocol consisting of sodium hypochlorite - saline wash - chlorohexidine with ultrasonic acctivation of the irrigation, obturation using warm vertical compaction technique and MTA putty to seal the coronal third perforations of the mesio-buccal canal and final restoration. The x-rays of the second visit shows slight extrusion of the intracanal medication beyond the apex and slight decrease in the size of the periapical radiolucensy. Follow up on the extending file is scheduled to determine if it needs to be surgically removed or if follow up is enough.
