Management of class II remains one of the most fun and challenging clinical situations specifically when trying to reproduce proper contact point/area. (NO SET RULES, BE CREATIVE!!!).

In this case the contact between the 1st and 2nd molars was too wide and missing both lingual and buccal distal cusps further complicated the case

1. Saddle matrix was used instead of normal sectional matrix as the contact was too wide.

2. Saddle holder was used in the first step to avoid pressure and collapse of the matrix while building the missing distal cusps.

3. A sectional matrix ring was then used to produce better and tighter contact.

4. The ring was positioned over the middle of the lingual wall instead of normal positioning, this is to
a) the 2nd molar is lingually positioned, so the ring would have adapted to it and left the matrix unadapted to the 1st molar lingually.
b) put pressure over the wedge to further the seal of the matrix over the gingival margin.

The main aim of practicing Biomimetics dentistry is to conserve as much as possible natural tooth structure (both on short and long terms).

To do so, a proper analysis of the tooth condition before the operation is mandatory. This analysis should include (pre-operative occlusal check) as it helps you determine which parts of the tooth are subjected to more stresses and which parts aren't.

In this case the enamel bridge between the buccal cusps of the lower first molar wasn't subjected to any occlusal forces. thus, even though the enamel was unsupported/undermined, we could preserve it and use Biomimetic materials to support it instead without fear of future failures.

First post in a while, a simple straight forward case of proper Biomimetics workflow.

As I always say, the key factors for predictable results are:
- proper analysis.
- proper updated evidence based knowledge.
- flexibility in applying the knowledge.

At first proper restorative work will be extremely slow and each step will take considerable time, but with persistent effort and adherence to the protocols, you start to accumulate experience and get faster eventually.
This was a good case for me to measure how much my speed has improved over the time since i started applying Biomimetics dentistry.
Typically it takes 2 visits to manage such a case, a 1st visit for the biobase and impressions and a 2nd visit for cementation. 2 years ago the first visit would've taken me 2 hours and 1 hour for the second one. But now, the 1st visit took me 45 mins and the second visit took 20 minutes.
This is a good reminder that our career requires practical experience, persistent hard work, studying and applying everyday, step by step until you get the hang of it.
Social media dentistry causes many young dentists to underestimate their capabilities and performance as it only shows the final top peak level of work, but it never shows the hard process and the bumpy road you need to take to reach that level.

APA (Air Particle Abrasion) has multiple clinical applications, those include but not limited to:
- caries removal.
- cavity preparation.
- plaque and stain removal.
- ceramics surface treatment before bonding.
- old composite removal.

Most commonly used types of powder are aluminium oxide (more aggressive for caries removal and preparations and surface treatment). and sodium bicarbonate powder (for cleaning and stain removal mainly).

Multiple studies have found that dentin surface treatment with aluminium oxide APA significantly INCREASE bond strength of adhesive systems yo the dentin (e.g. Sinjari et al 2020 , mavriqi et al 2021)

In this case the APA was done by aquacare from .egypt using aluminum oxode powder 53u

The main goal of Biomimetics dentistry is VITALITY PRESERVATION. Even if the caries is reaching the pulp does that mean endotreatment is mandatory? The short answer is no, as long as the pulp is vital and responds to vitality tests properly then there's still hope for the tooth.
A case of extremely deep 2ry caries, management iof such cases is both challenging and fun specifically the part of establishing CRE (caries removal endpoint).
Also it is highly dependent on proper structural analysis, in this situation both cusps were thinned and undermined so we decided to go for indirect cuspal coverage.

Quadrant management with both deep margins and extremely deep caries management. In such cases patient communication is a very important key.

In dental practice for every clinical situation we must provide the patient with more than one treatment plan and allow them to take part in deciding which treatment they would prefer. Our role as dentists is to provide knowledge, diagnosis and prognosis, to guide the patient but not to force them.

For this case i would have preferred to do an indirect restoration for the 1st premolar, however the patient decided to go for a direct solution.
