Bruxism Highyield Points ⬇️

Definition
🔹 Parafunctional grinding/clenching/rubbing of teeth
🔹 Can be awake or during sleep

Types
🔹 Awake bruxism: stress/anxiety-related clenching
🔹 Sleep bruxism: centrally mediated sleep-related movement disorder

Etiology
🔹 Central factors most important: stress, anxiety, psychosocial factors, sleep micro-arousals
🔹 Drugs: SSRIs (fluoxetine, sertraline), amphetamines, co***ne
🔹 Associated conditions: OSA, Parkinson disease, GERD
🔹 Occlusion is not a primary cause

Clinical features
🔹 Teeth: attrition/wear facets, fractures, hypersensitivity, abfraction, ↓ VDO
🔹 Muscles: masseter hypertrophy → square jaw, morning pain/fatigue
🔹 TMJ: pain, clicking, popping, limited opening
🔹 Soft tissue: linea alba, scalloped tongue

Management
🔹 Goal: protect teeth, reduce muscle activity
🔹 Hard acrylic night guard
🔹 Soft splints usually avoided
🔹 Stress control, biofeedback, relaxation, sleep hygiene
🔹 Severe cases: cyclobenzaprine short term or Botox
🔹 Suspect OSA → sleep study; suspect SSRI-induced → physician review

High-yield pearl
🔹 SSRI + new bruxism = classic board association
🔹 Square jaw + wear facets + scalloped tongue = bruxism

This is what an untreated cavity really looks like — from the inside.

The dark area at the top is advanced dental caries, where bacteria have destroyed the enamel and dentin. Beneath it lies the pulp chamber, rich in nerves and blood vessels — shown here inflamed and exposed.

Once decay reaches this depth, pain is no longer just “toothache.” It is nerve infection, inflammation, and a gateway for bacteria to enter the bloodstream.

At this stage, a simple filling is no longer enough. Treatment usually requires root canal therapy or extraction — both more complex, costly, and invasive than early care.

Inflammation ét sted i kroppen nærer inflammation andre steder i kroppen. Parodontitis giver lavgraderet systemisk inflammation i kroppen.

Brushing before bed does more than keep your mouth clean — it may also support your heart. Overnight, oral bacteria multiply and can slip into the bloodstream, triggering inflammation that plays a role in heart disease.

Maintaining a nightly brushing habit lowers bacterial buildup, keeping gums healthier and reducing the chance of these microbes entering circulation. A cleaner mouth means less internal stress on the heart.

Research from the National Library of Medicine links consistent nighttime brushing with a lower risk of cardiovascular complications. A small daily habit could contribute to stronger long-term heart health.

🦷 New Study Finds Oral Bacteria Inside Heart Arteries — Suggesting a Direct Link Between Dental Biofilm and Heart Attacks 🦠🫀

A recent study published in the Journal of the American Heart Association (August 2025) has identified DNA from oral bacteria—specifically Viridans streptococci—within nearly half of examined coronary artery plaques. Researchers analyzed arterial samples from patients who had suffered heart attacks and discovered that these bacteria were organized as biofilms, not free-floating cells.

Biofilms are dense bacterial communities encased in a protective matrix. Once formed, they can evade immune defenses and persist for long periods. The study suggests that when these biofilms are disturbed, they may trigger localized inflammation and plaque rupture, both critical events that can lead to myocardial infarction.

This research strengthens the growing evidence that poor oral hygiene and untreated gum infections may have systemic consequences far beyond the mouth. While a direct cause-and-effect pathway is still being studied, the findings highlight how oral microorganisms can infiltrate the bloodstream and integrate into vascular tissues—potentially linking chronic dental biofilm with cardiovascular disease progression.

The study underscores an emerging scientific perspective: the mouth may serve as an entry point for bacteria involved in heart pathology, redefining how both dental and cardiovascular health should be understood in the context of whole-body disease prevention.
