NUG can cause tissue destruction involving the supporting structures. It usually runs an acute course and therefore
the term acuteis often included in the diagnosis.
When bone loss occurs the condition is called necrotizing ulcerative periodontitis.
If left untreated infection reaches into systemic circulation, and may spread to other parts of the body.

If ANUG left untreated, it may spread laterally and apically to involve the entire gingival complex, including the mucosa and alveolar bone,
leading to necrotizing ulcerative periodontitis to necrotizing ulcerative stomatitis and finally Noma.

Index suggested by Prichard (1972) :
1. Slight mobility
2. Moderate mobility
3. Extensive movement in a lateral or mesiodistal direction combined with vertical displacement in the alveolus.
Plus and minus sign can be used for added refinement.

Index given by Waserman et al, (1973)
1. Normal
2. Slight mobility: Less than approximately ¾ mm (295X10 ) of bucco-lingual movement.
3. Moderate Mobility: Up to approximately 2 mm (790X10 ) movement bucco-lingually.
4. Severe Mobility: More than 2 mm (790X10 ) of movement.

Tooth mobility is considered to be significant when evaluating the effectiveness of periodontal therapy. The etiology of mobility has been attributed to either a reduction in the resistance of the teeth or to an accentuation of the magnitude of the forces placed upon them. The significance of mobility, however, is not clear. Since many of the therapeutic modalities employed in its treatment have negative aspects, the decision to treat or not to treat mobility should be based upon & critical evaluation of the possible advantages and disadvantages of the proposed therapy.

This video shows effects of improper forces on teeth , when your bite is slightly off one tooth may hit sooner than the rest this causes undue stress on the involved teeth and then they begin to flex, over time this continual flexing and stress cause enamel to separate from the inner dentin layer of the tooth forming a notch at the gum line.
