We are excited to see our patients again. To ensure your safety and the safety of our staff, please review the important changes before your next appointment.

*THERE HAVE BEEN SOME CHANGES*

-You must wear your own mask to the appointment.
(If you do not have a mask, please call the office in advance and a mask can be provided to you for a fee of $1.)
-Please bring your own pen to fill out any documents.
-Only the patient with the appointment are allowed in the office. If a legal guardian or interpreter must accompany the patient, please notify our front desk when booking the appointment so they can fill out our screening questions as well.
-Contactless payment methods are preferred e.g. tap with Debit or Credit Card.
