This patient did not like the space that he had in between the front teeth. Notice also that the edges of the two front teeth are different lengths. ⁣

Bonding corrected both problems. The procedure is done in one visit and does not require anesthesia. The space is carefully closed so that the colour and the contour of the bonding mimic natural structure. When the restorations are polished, they can have the same shine as enamel making them unnoticeable. ⁣

Our patient presented with worn and chipped front teeth. There was significant wear behind the front teeth due to a deep bite. To restore his front teeth, he had to see the orthodontist so we could establish a proper relationship between the upper and lower front teeth. After the orthodontic treatment was completed, we whitened the teeth, placed four ceramic crowns on the upper teeth and placed dental bonding on the lower teeth. The patient’s bite was harmonized so that the jaw joint, muscles, and teeth are all working in coordinated manner. ⁣

Technical notes: The wear pattern for this patient was localized to the anterior teeth with significant lingual wear that did not allow for restorative space to restore the teeth in a conservative manner. Orthodontics was the first step to create restorative space in the anterior region. The photos shown are all post-orthodontics. With enough restorative space in the anterior, the lower anterior teeth were restored in resin to create an ideal mandibular incisal plane. Then the maxillary anterior teeth (laterals and centrals) were restored with Emax crowns to restore ideal incisal length, and to establish anterior guidance. The patient was equilibrated to centric relation to allow for even contacts on all teeth with the condyles in a fully seated position and immediate disclusion with all mandibular movements. The gingival architecture would be a concern with a patient with a high smile, but not in this case and the patient was not bothered by this. The goals were to try to harmonize aesthetics and function with the least amount of dentistry as possible.⁣

This patient was fustrated because the bonding that she had done elsewhere was beginning to discolour, stain and chip. She had the treatment done just over 1 year ago. Although bonding can wear and stain over time, we would expect a lifespan of 5-7 years before a touch up may be required. ⁣⁣
⁣⁣
The good news is that it is easily refreshed. The old bonding was removed, the tooth structure underneath remains intact and the new bonding is meticulously placed and finished. ⁣⁣

Ensuring that a child’s first visit is a good experience can really set the stage of establishing a good relationship with regular dental care in the future. Negative dental experiences when one is young can be imprinted to the point that many adult patients are fearful of dental care. ⁣
⁣
We strive to make the first visit positive. We have a little ride in the chair. We introduce instruments by counting the teeth. We do a polish with our “special toothbrush”. All this to make sure our child patient understands what we are doing in relatable actions. ⁣

When all goes well, both dentist and patient have a smile at the end of the visit! 😀⁣

Aesthetics and function of teeth need to be considered together. When we have chipping and wear of the front teeth, grinding is often the cause. However, sometimes the teeth are in a position whereby they just get in the way of chewing and speaking. We notice tooth wear in these circumstances as well. Aligning the teeth so that they are in a better position (swipe➡️) and then addressing the chipped or worn front teeth will allow our bonding (in this case) to last and serve out patient for as long as possible. ⁣⁣

Capable, intelligent, hardworking and all around good guy, he has taken his skills to Sinclair and is now a service technician. Sometimes the unfortunate happens! Our patient took a tumble from a ladder. Thankfully, the injuries were not extensive. There was no exposure of the nerves from the tooth fractures and all we needed was a 1 visit appointment to get our patient smiling again! ⁣
