๐ The patient was referred to us for evaluation and extraction of this tooth. She had been struggling with pain in this tooth for ๐ฆ๐จ๐ซ๐ ๐ญ๐ก๐๐ง ๐๐ ๐ฒ๐๐๐ซ๐ฌ , constant discomfort that no one could ever diagnose properly.
X-rays and exams over the years didnโt reveal much, and she was simply told to โmonitorโ it. But the pain never went away.
When she came to us, we took a closer look โ and found the real culprit: ๐ ๐๐จ๐ซ๐ ๐จ๐ญ๐ญ๐๐ง ๐๐ซ๐จ๐ค๐๐ง ๐๐ข๐ฅ๐ ๐ฅ๐๐๐ญ ๐๐๐๐ฉ ๐ข๐ง๐ฌ๐ข๐๐ ๐ญ๐ก๐ ๐ซ๐จ๐จ๐ญ ๐๐๐ง๐๐ฅ ๐ฒ๐๐๐ซ๐ฌ ๐๐ ๐จ.
Over time, that small piece of metal had caused a chronic inflammatory reaction, preventing proper healing and leading to progressive apical root resorption.
When a file breaks during endodontic treatment and remains inside the canal, it can block full cleaning and disinfection. Bacteria stay trapped inside, keeping the periapical tissues constantly inflamed. The body reacts to this chronic irritation, slowly breaking down the bone and root structure around the apex.
If the file had been identified and treated earlier, this tooth could have been saved through retreatment or apical surgery. But after years of ongoing infection and resorption, the damage was too advanced โ leaving extraction as the only possible solution.
