Sometimes a canal can appear almost invisible on an X-ray due to calcification — but that does not mean we give up on the tooth!

Challenging Anatomy + Healing in Progress 🦷

#26 presented with:
• Significant canal calcification
• Pronounced S-shaped curvature in DB canal
• Apical pathology

Despite the difficulty negotiating and shaping this anatomy, canals were located, negotiated, and treated to full working length.

At 6 month follow-up, we can observe:

• Well-adapted obturation following the natural canal curvature
• Respect of the original anatomy
• near complete healing of the apical tissues

#27 is currently undergoing treatment- stay tuned!

These cases are always a good reminder—patience, magnification, and respect for anatomy are everything.

Case Spotlight: Internal Resorption & Complex Anatomy

At Carling Endodontics, advanced imaging plays a critical role in diagnosis and treatment planning.

This case presented with internal resorption in a mandibular molar, combined with unusual canal anatomy—a scenario where conventional 2D radiographs alone may not provide the full picture.

Using CBCT (3D imaging), we were able to:
✔️ Clearly identify the extent and location of the resorptive defect
✔️ Visualize complex root canal anatomy
✔️ Accurately plan and execute treatment with confidence

The 3D assessment allowed for a more predictable outcome, ensuring all anatomical variations were addressed and the resorptive defect was properly managed.

Endodontics is not always about devitalizing the tooth. This case is a great example of how conservative and careful management of vital tissues will maintain vitality and allow continued root developemet:

✅ Step 1: Decay and inflamed tissue were carefully removed.
✅ Step 2: A protective bioceramic material was placed to promote healing.
✅ Step 3: The tooth was sealed and restored — preserving vitality and strength.

Follow-up radioraph shows a dentin bridge formation and continued root formation.

This patient presented to us with history of signoficant swelling and a very large periapical radiolucency. We treated #22 over the course of one year with several rounds of intracanal medication. We see great bone regenration after 1 year. This was achieved without any surgical intervention.

S- shaped curvatures on a lowe molar. This anatomy is very delicate and could lead to file fracture.
