Oral Drug Pathway

Mouth → Esophagus → Stomach → Small Intestine (absorption) → Portal Vein → Liver (first-pass metabolism) → Systemic Circulation → Target Tissue → Metabolism → Excretion

Key Points:

* Most drugs are absorbed in the small intestine.
* Drugs absorbed from the GI tract enter the portal circulation and pass through the liver first.
* First-pass metabolism may reduce bioavailability before the drug reaches systemic circulation.
* Drug metabolism occurs mainly in the liver via Phase I and Phase II reactions.
* Excretion occurs primarily through the kidneys.

ADME = Absorption → Distribution → Metabolism → Excretion.

💉 Why do patients on long-term steroids need increased steroid dose during surgery?

Chronic steroid use suppresses the body’s normal cortisol production (HPA axis suppression). During surgery, the body faces major stress and normally releases extra cortisol to maintain blood pressure, glucose levels & stress response.

⚠️ If supplemental steroids are not given, the patient may develop an adrenal crisis causing:
• Severe hypotension
• Shock
• Hypoglycemia
