Why does my prosthetic socket start feeling loose?

A prosthetic socket that once fit comfortably can gradually start to feel loose. This is especially common during the first year following an amputation.

Why does this happen?

Your residual limb is constantly changing. As you progress through rehabilitation, several things can affect its size and shape:

- Swelling decreases as your limb heals
- Residual-limb volume decreases as the limb matures
- Muscle atrophy and changes in muscle mass can alter its shape
- Fluid levels can fluctuate throughout the day
-️ Changes in body weight can also affect socket fit

As your residual limb gets smaller, there is more space between your limb and the socket.

You may start noticing that you're wearing more prosthetic socks, your limb is moving up and down inside the socket, the prosthesis feels less secure, or you're developing new areas of pressure or discomfort.

Small day today changes can often be managed by adding or removing prosthetic socks to maintain an appropriate fit.

But if you're consistently needing more socks or your prosthesis no longer feels comfortable and secure, it's time to have your socket assessed by your prosthetist.

Sometimes an adjustment is all that's needed. Other times, the changes to your residual limb may be significant enough that a replacement socket is required.

Your residual limb changes. Your socket fit can too.

More than a prosthesis. More ways to play.

Whether it’s taking a shot, practising gymnastics, doing a push up, or trying something new, childhood is full of opportunities to play, explore, and grow.

Different activities call for different support. A prosthesis can be designed around a child’s interests, helping them take part in the things they love in their own way.

At Hamilton Prosthetics & Orthotics, we work with children and their families to support their goals as they grow, play, and discover what comes next.

What happens after I finish rehabilitation and need my definitive prosthetic socket?

One of the biggest changes following an amputation happens during the first year and it isn't just learning how to use a prosthesis.

Your residual limb is changing too.

As swelling decreases and the residual limb matures, its shape and volume can change significantly. This can cause a prosthetic socket that initially fit well to gradually become loose.

That's why your first prosthetic socket is considered temporary.

In Ontario, ADP may provide funding for up to three temporary socket replacements during the first year following your initial prosthetic fitting, when clinically required. These replacement sockets help accommodate the changes that can occur as your residual limb matures.

Eventually, those changes typically begin to stabilize.

Once your rehabilitation has progressed and your residual limb has reached a more stable size and shape, your prosthetic team may determine that you're ready to transition from a temporary prosthesis to a definitive prosthesis.

A definitive socket is designed for longer term use and provides an opportunity to optimize your fit, comfort, suspension and overall prosthetic system based on what we've learned throughout your rehabilitation.

The temporary prosthesis gets you started.
The definitive prosthesis is the next stage of your prosthetic care.

What actually happens when someone gets their first prosthesis?

For a new amputee, receiving a first prosthesis involves much more than simply choosing and fitting a prosthetic leg.

It starts with an assessment in office or in amputee clinic at our regional rehabilitation hospital . We learn about the person’s mobility, goals, daily activities and rehabilitation needs.

From there, the process may include:

- Assessment / prescription
- Funding approval
- Casting or digital scanning to capture the residual limb
- Socket design and fabrication specific to the individual
- Diagnostic fittings to evaluate comfort, fit and suspension
- Alignment and walking trials to optimize stability and function
- Delivery and rehabilitation to learn how to safely use the prosthesis
- Follow up and adjustments as mobility improves and the residual limb changes

One of the most important things for new amputees and their families to understand is that receiving the prosthesis isn't the end of the process.

Prosthetic care is ongoing. Fit, alignment and components may need to be adjusted as the person progresses through their rehabilitation.

Every prosthesis is different because every person is different.

What’s the difference between a $5,000 and a $50,000+ prosthetic knee?

The biggest difference is the technology inside the knee and how it responds to the person using it.

A conventional mechanical or hydraulic knee can provide reliable control and work extremely well for the right person.

A microprocessor knee uses sensors and onboard computers to monitor movement and continuously adjust resistance while the person walks. Depending on the system, this can provide enhanced stumble recovery, improved control on slopes and stairs, and better adaptation to changes in walking speed.

But here’s the important part:

The most expensive knee isn’t automatically the best knee.

The right prosthetic knee depends on the individual’s mobility, goals, environment, health, safety needs and daily activities.
