Let’s talk about pterygoid dental implants.

I’ve shown cases in the past where zygomatic implants have been placed for severely atrophic upper jaws. Pterygoid implants are another example of remote anchorage that allow implant placement for patients with reduced bone levels in the upper jaw. These pterygoid implants are placed into the dense bone of the pterygomaxillary junction to allow for a distal anchorage point on a fixed dental prosthesis.

Having an implant in the pterygoid region allows for a full set of teeth up to the second molars since cantilevers are eliminated. For patients with expanded maxillary sinuses, there may only be enough bone for four implants near the front of the jaw, and this requires a cantilever extension to have teeth in the premolar or molar region.

In dentistry, the longer the cantilever, the more problems you have. Adding pterygoid implants circumvents the limitations presented by expanded maxillary sinuses and provides a more stable, durable prosthesis that will last the test of time.

Here is the 3D radiograph for a recent patient of mine that had six lower implants and six upper implants - two of which were pterygoid implants.

Here is a case where tooth #22 was removed and an implant was placed right away. For suitable candidates, this is an excellent way to save about 4-months of healing time, meaning the patient will have their final crown in place sooner. A prosthetically-driven surgical approach is utilized to ensure a successful result with these cases.

During the 4-months of healing after surgery, a flipper, or temporary partial denture, is worn.
