Whistle while you work Wednesday.....An interesting case indeed. There is a fractured file in the MB canal, a cementoblastoma at the apex of the distal root, and the canal in that root is wide in the B-L direction with a wafer-thin furcal wall.
I was able to bypass the file in the MB canal; there is a small mountain of hand files to prove that. I had to treat the distal root as though it had three unique canals, trying my best to seal the apical constriction that is B-L and not create a strip perforation, too.
The cementoblastoma is something that has remained unchanged for years.
