Tooth 36 Retx. This case was treatment planned for an extraction. Patient came in for a second opinion. CBCT shows missed DL and 9mm B furcation pocket. Proceeded with two stage ReTx using Diapex. 3 month finish appointment had no pocket and early signs of radiographic healing. Understanding the pathology, history, and CBCT findings is key.

Tooth 36 was treatment planned for an extraction due to the large J shaped lesion and narrow pocket. CBCT shows a break in the buccal plate coronally but no other disruption of the B/L plates at the mid/apical root level - previous endo was also not adequate. Decided to proceed with an n-step Retx to salvage this molar. After 4 months of CaOH/re-rinsing, the buccal pocket (Sinus tract through sulcus) was resolved and radiographic healing can be observed at the completion.

Tooth 16 Retx. Initial PA with traced sinus buccal parulis looks unfavorable due to the extrusion of material from the P root. CBCT shockingly showed no pathology or PDL widening on the P root. Lesion seen around the MB/DB root along with a missed MB2. Crown was dislodged during treatment, distal caries were removed, and the tooth was restored. 3 week follow up showed resolution of the parulis and the case was completed.

Sometimes the RCT is not needed. 11-year-old patient with no endodontic symptoms. Restored with an indirect pulp cap using bioceramic putty. This tooth will likely need RCT in the future, but it is always better to delay given the patient's age.
