🦷 Tooth #26 - Before & After Root Canal Retreatment by ._bakhtiar

This case involved Tooth #26, which was symptomatic with an inadequate previous root canal treatment. The patient reported intermittent pain, and the X-ray revealed the complexity of the anatomy—curved canals and a distal split that conventional treatment would struggle to fully clean.

Unlike traditional rotary instrumentation alone, GentleWave uses multistage acoustic energy + hydrodynamic fluid dynamics to clean deep into the curved and hard-to-reach branches of the root canal system.

✔️ NaOCl was used for tissue dissolution
✔️ EDTA removed the smear layer
✔️ A SoundSeal platform was built to ensure proper delivery
✔️ The system circulated fluid continuously to enhance debridement and disinfect even the tiniest crevices

This approach is ideal for anatomies like this—where there’s a split in the apical third or sharp curves that rotary files simply can’t reach.

After thorough disinfection, the canals were dried and obturated using BC Sealer + gutta-percha, then sealed with a purple flowable composite and IRM for temporary restoration.

🦷✨ Surgical Spotlight: Cystic Lesion Removal! ✨🦷

We had an interesting case involving a patient who came in with persistent pain and numbness in the lower right area. After a thorough evaluation, including a panoramic X-ray, we identified a cystic lesion—our differential diagnosis pointed toward a Keratocystic Odontogenic Tumor (KOT).

To get started, we performed an extraction of tooth #48, which was severely decayed. This allowed .daniel.fortino to access the cyst effectively. Once we removed the cyst, the inferior alveolar nerve (IAN) was clearly visible, allowing us to proceed safely.

After debridement, we utilized an Amnion Chorion membrane, combined with iodoform packing gauze and 5-FU. The Amnion Chorion membrane helps promote healing by providing a scaffold for tissue regeneration, while the iodoform gauze helps to control any potential infection. 5-FU was included to assist in reducing the chance of recurrence by inhibiting cellular proliferation in the area.

After carefully suturing the site, we wrapped up the procedure and sent the cyst off for a biopsy to confirm our clinical diagnosis.

We’re optimistic about a smooth recovery for our patient and await the Biopsy Results
