Dr. Kara recently completed root canal treatment on tooth #31 which presented with some unique complex anatomy (Dx: PN, SAP). Radiographic analysis showed a main mesial canal and then a separate distal infolding penetrating through the root with its own apical foramen. Based on Oehlersโ classification (1957), this appears to be a Type IIIb dens invaginatus (DI). The DI had an open apex, therefore apexification was performed and it was sealed apically with bioceramic putty. Teeth can have such variable anatomy!
