This patient presented with advanced periodontal disease, a high smile line (gummy smile), and a severely compromised maxillary dentition.

A high smile line is one of the greatest esthetic challenges in full-arch implant rehabilitation, as the prosthesis–tissue junction may be exposed during smiling. Without careful planning, this can lead to visible transitions, unnatural soft tissue contours, phonetic issues, and compromised esthetic outcomes.

Our philosophy is simple: preserve natural teeth whenever possible.

Before implant rehabilitation, the patient underwent comprehensive periodontal therapy to stabilize and improve the remaining mandibular dentition, allowing preservation of healthy lower teeth instead of unnecessary full-mouth extraction.

The final rehabilitation included:
✅ Maxillary All-on-6 fixed full-arch restoration with a milled titanium bar supporting monolithic zirconia teeth
✅ Two posterior mandibular implant-supported segmental bridges, preserving the patient’s natural anterior teeth

Using the Chrome Guided Surgery system, implants were placed with prosthetically driven precision. Nexus IOS digital impressions ensured an accurate passive prosthetic fit, contributing to predictable long-term function and esthetics.

This case highlights that successful implant dentistry isn’t about replacing every tooth—it’s about combining periodontal therapy, digital technology, and meticulous planning to preserve what can be saved while restoring what has been lost.

Orthodontic treatment starts with a healthy periodontal foundation.

This patient presented with multiple abnormal frenum attachments and an inadequate width of keratinized tissue (KT), creating excessive tension on the gingival margins and increasing the risk of soft tissue instability during orthodontic treatment.

To establish a healthier foundation, we performed mucogingival surgery with a Free Gingival Graft (FGG) before orthodontics to increase the band of keratinized tissue and improve the quality and stability of the soft tissue.

➡️ Before: Multiple high frenum attachments with thin, inadequate keratinized gingiva, increasing the potential for plaque accumulation, inflammation, and gingival recession.

➡️ After: A wider, healthier zone of keratinized tissue with reduced soft tissue tension, providing a more stable periodontal environment for orthodontic tooth movement.

Soft tissue matters. While orthodontics aligns teeth, the surrounding gingiva plays a critical role in protecting against recession and helping preserve the supporting periodontal tissues. Careful interdisciplinary planning between the periodontist and orthodontist helps create healthier, more predictable, and long-lasting outcomes.

Complex implant reconstruction is about much more than replacing a missing tooth. It requires rebuilding lost bone, recreating soft tissue architecture, and carefully managing every detail to achieve a natural, long-lasting result.
