🔎 কীভাবে হয়?
সাধারণত নিচের কারণে pulp necrosis হয়ঃ
গভীর ডেন্টাল ক্যারিজ
ট্রমা (দাঁতে আঘাত)
দীর্ঘদিন untreated pulpitis
বারবার restorative procedure

🦷 কী ঘটে?
প্রথমে Reversible pulpitis → Irreversible pulpitis → Necrosis
Blood supply বন্ধ হয়ে pulp tissue মারা যায়
ব্যাকটেরিয়া root canal দিয়ে periapical area পর্যন্ত ছড়াতে পারে

⚠️ লক্ষণ
অনেক সময় ব্যথা থাকে না (tooth becomes non-vital)
কালো বা ধূসর রঙ হয়ে যেতে পারে
Percussion tenderness থাকতে পারে
Periapical abscess / swelling হতে পারে
Sinus tract দেখা যেতে পারে

🧪 Diagnosis
Pulp vitality test (cold test, EPT) → negative
Radiograph → periapical radiolucency থাকতে পারে

💊 চিকিৎসা
Root Canal Treatment (RCT) – tooth save করার জন্য
যদি tooth non-restorable হয় → Extraction

🦷 Dental Fluorosis (ডেন্টাল ফ্লুরোসিস)

Dental fluorosis হলো দাঁতের এনামেলের বিকাশকালীন সমস্যা, যা হয় যখন শিশুকালে (০–৮ বছর বয়সে) অতিরিক্ত fluoride গ্রহণ করা হলে।

🔎 কারণ (Cause)
পানির মধ্যে বেশি fluoride (ground water)
অতিরিক্ত fluoride toothpaste গিলে ফেলা
Fluoride supplement বেশি খাওয়া
Industrial fluoride exposure

🧬 কিভাবে হয়?
দাঁতের এনামেল তৈরি হয় Ameloblast কোষ দিয়ে।
অতিরিক্ত fluoride → ameloblast এর কাজ ব্যাহত করে → enamel mineralization ঠিকমতো হয় না → hypomineralized enamel তৈরি হয় → fluorosis দেখা দেয়।

📊 Dean’s Index অনুযায়ী Classification
Questionable – সামান্য সাদা দাগ
Very Mild – ২৫% এর কম opaque white area
Mild – ৫০% এর কম
Moderate – ব্রাউন স্টেইন + রাফ সারফেস
Severe – পিটিং, ব্রাউন দাগ, এনামেল ভেঙে যাওয়া

👁️ Clinical Features
চকচকে সাদা দাগ (white opaque lines)
Chalky white patch
Brown stain (moderate–severe case)
Surface pitting (severe case)
⚠️ সাধারণত bilateral এবং symmetrical হয়।

Bleaching
Microabrasion
মডারেট–সিভিয়ার:
Composite veneer
Porcelain veneer
Crown (severe case)

🛡️ প্রতিরোধ
পানির fluoride level 0.7 ppm রাখা
৬ বছরের কম বাচ্চাকে pea-sized toothpaste দেওয়া

🦷 Enamel Hypoplasia

Enamel hypoplasia হলো দাঁতের এনামেলের (সবচেয়ে বাইরের শক্ত স্তর) পরিমাণগত ত্রুটি (quantitative defect) — অর্থাৎ এনামেল কম তৈরি হয় বা কিছু অংশে একদমই তৈরি হয় না।
এটা হয় যখন ameloblast কোষ এনামেল গঠন করার সময় কোনো সমস্যা হয়।

🔎 কারণ (Causes)

🧒 Local causes (একটি/কয়েকটি দাঁতে)
Trauma (দুধ দাঁতে আঘাত → স্থায়ী দাঁতে প্রভাব)
Infection
Periapical inflammation (Turner’s tooth)

🌍 Systemic causes (অনেক দাঁতে)
Malnutrition (Vitamin A, C, D deficiency)
Calcium deficiency
Low birth weight / Prematurity
High fever during childhood
Congenital diseases
Genetic cause → Amelogenesis imperfecta

👀 ক্লিনিক্যাল বৈশিষ্ট্য (Clinical Features)
Enamel পাতলা বা অনুপস্থিত
Pits / grooves / linear defects
Rough surface
Yellowish appearance (কারণ dentin দেখা যায়)
সহজে caries হয়
Sensitivity থাকতে পারে

🛠️ চিকিৎসা (Treatment)
ক্ষতির পরিমাণ অনুযায়ী:
Mild → Composite restoration
Moderate → Veneer
Severe → Crown
Sensitivity হলে → Desensitizing agent

- Requires surgical removal

🔍 Mucocele:
- Lesion on oral mucosa

Silver diamine fluoride (SDF)

Silver diamine fluoride (SDF) হলো একটি টপিক্যাল ডেন্টাল মেডিকেশন যা দাঁতের ক্যারিজ (tooth decay) থামাতে ব্যবহার করা হয়—বিশেষ করে শিশু, বৃদ্ধ ও বিশেষ চাহিদাসম্পন্ন রোগীদের ক্ষেত্রে।

🔬 গঠন
Silver (Ag) → অ্যান্টিব্যাকটেরিয়াল
Fluoride (F⁻) → রিমিনারালাইজেশন করে
Ammonia → সলিউশনকে স্থিতিশীল রাখে
সাধারণত 38% SDF (প্রায় 44,800 ppm fluoride) ব্যবহার করা হয়।

✅ ইন্ডিকেশন
Early childhood caries
Root caries (বৃদ্ধদের ক্ষেত্রে)
অস্থায়ীভাবে ক্যারিজ আটকাতে (interim management)
সহযোগিতা না করা শিশু (uncooperative child)
মেডিক্যালি কম্প্রোমাইজড রোগী

⚙️ কাজ করার পদ্ধতি
ব্যাকটেরিয়া ধ্বংস করে
ডেন্টিনাল টিউবিউল অবরুদ্ধ করে
রিমিনারালাইজেশন বৃদ্ধি করে
ক্যারিজের অগ্রগতি থামায় (arrest)

🦷 ব্যবহার পদ্ধতি (সংক্ষেপে)
Isolation
Carious lesion শুকানো
Microbrush দিয়ে SDF প্রয়োগ (১–৩ মিনিট)
Air dry
বছরে ২ বার প্রয়োগ করা যায়

⚠️ অসুবিধা
ক্যারিজ অংশ কালো হয়ে যায় (স্থায়ী দাগ)
Soft tissue এ লাগলে সাময়িক দাগ
Rarely mild irritation
