Immature Permanent Tooth 🦷

A 9 year old patient presented with a
vital immature maxillary first permanent
molar (UR6).

Following coronal pulp removal,
MTA was placed over the remaining vital
radicular pulp,followed by a temporary
restoration. The palatal root showed incomplete
root development with an open apex
making preservation of pulp vitality especially
important for continued physiological root
development (apexogenesis).

Calcified Canal Negotiation 🦷
From Initial Access to Obturation.

Calcified canal সবসময় শুধু একটি technical challenge নয়; এটা patience, tactile sense এবং controlled instrumentation এরও পরীক্ষা।

এই case এ canal টি significantly calcified ছিল। Access করার পর chamber floor ভালোভাবে পরিষ্কার করে canal or***ce এর সম্ভাব্য অবস্থান ও direction carefully assess করি। শুরুতেই বেশি dentin remove
না করে conservative approach অনুসরণ করেছি।

Initial Canal Negotiation:
প্রথমে #06 এবং #08 pre-curved K-file দিয়ে খুব gentleভাবে canal pathway খোঁজার চেষ্টা করি।
যেখানে resistance পেয়েছি, সেখানে force প্রয়োগ না করে file বের করে curvature adjust করে আবার negotiate করেছি। EDTA/chelating lubricant ব্যবহার করে frequent irrigation ও recapitulation করেছি।

ধীরে ধীরে #06 → #08 → #10 K-file-এর মাধ্যমে
canal pathway establish করতে সক্ষম হই। #10 file দিয়ে একটি reproducible glide path পাওয়ার পর mechanical preparation এর দিকে যাই।

Working Length:
Canal negotiate হওয়ার পর apex locator-এর
মাধ্যমে working length determine করি এবং RVG
দিয়ে file position cross check করি। Calcified canal হওয়ায় working length নিশ্চিত করার ক্ষেত্রে tactile feedback এর পাশাপাশি electronic ও radiographic confirmation দুটোকেই গুরুত্ব দিয়েছি।

Canal Preparation:
Glide path establish হওয়ার পর controlled rotary instrumentation এর মাধ্যমে canal prepare করি।
পুরো preparation এর সময় frequent irrigation এবং recapitulation বজায় রেখেছি, যাতে debris accumulation এর কারণে তৈরি হওয়া pathway
আবার বন্ধ না হয়ে যায়।

Irrigation & Final Preparation:
Instrumentation এর প্রতিটি ধাপে পর্যাপ্ত irrigation
বজায় রেখে canal পরিষ্কার করেছি। Final preparation এর পর canal thoroughly irrigate করে obturatio
এর জন্য প্রস্তুত করি।

Obturation:
Canal শুকিয়ে নিয়ে appropriate gutta percha cone
fit ও radiographic assessment-এর পর obturation সম্পন্ন করি। শেষে Bio-C Sealer ব্যবহার করে canal system seal করেছি এবং obturation এর final radiographic result evaluate করেছি।

এই case-এর সবচেয়ে বড় learning আমার কাছে
Calcified canal এ force নয়, patience এবং canal anatomy follow করাটাই বেশি গুরুত্বপূর্ণ।

ছোট pre curved hand file, controlled negotiation, proper chelation, frequent irrigation, recapitulation এবং একটি reproducible glide path এই basic steps গুলোই challenging calcified canal management এ অনেক difference তৈরি করতে পারে।

Clinical experience থেকে পাওয়া এই approach
ও practical tips share করলাম। আশা করি challenging calcified canal এর ক্ষেত্রে কারও কাজে আসবে।

একটি Endodontic Case
যেখানে প্রতিটি ধাপের গুরুত্ব রয়েছে...🦷

এই কেসে Working Length নির্ধারণ,canal negotiation, biomechanical preparation এবং obturation সম্পন্ন করা হয়েছে যথাযথভাবে। Post-operative radiograph
এ obturation এর পর সামান্য unintentional sealer extrusion beyond the apical foramen লক্ষ্য করা যায়।

এ ধরনের extrusion অনেক সময় obturation
এর সময় sealer এর flow, apical anatomy এবং apical pressure এর সমন্বিত প্রভাবে হতে পারে। যদিও extrusion টি ইচ্ছাকৃত ছিল না, radiograph review এর মাধ্যমে বিষয়টি identify করা এবং clinical context অনুযায়ী evaluate করা অত্যন্ত গুরুত্বপূর্ণ।

এই case-এর সবচেয়ে বড় learning point হলো
Working Length যতই accurately determine করা হোক, apical anatomy এবং obturation dynamics এর প্রতি constant awareness থাকা প্রয়োজন।

Clinical dentistry তে প্রতিটি case শুধু একটি
treatment নয়; প্রতিটি case পরবর্তী treatment কে আরও precise, controlled এবং predictable করার সুযোগ। Small extrusion, big lesson. 🦷

Calcified canal বিশেষ করে Upper 21 এর মতো
সরু ও partially obliterated canal manage করা
সবসময় straightforward নয়।

এই case এ initial attempts এ desired working
length পাওয়া যাচ্ছিল না। Canal negotiation এর
প্রতিটি ধাপেই ছিল যথেষ্ট technical challenge কিন্তু
হাল ছেড়ে না দিয়ে repeated assessment, careful negotiation এবং radiographic correlation
এর মাধ্যমে শেষ পর্যন্ত accurate working length determine করা সম্ভব হয়েছে।

এরপর যথাযথ canal preparation ও three-dimensional obturation সম্পন্ন করা হয়েছে।

সব case প্রথম চেষ্টাতেই ideal result দেয় না।
কিছু case আপনাকে একটু বেশি patience, precision আর persistence দাবি করে।
