Impacted wisdom teeth are third molars that don't have enough space to fully erupt in the mouth. Because of this, they often grow at odd angles — pushing against the second molars. What’s alarming is that this damage usually happens without any noticeable pain in the early stages. You might feel completely fine, while silent destruction is taking place underneath the gums.

As the impacted wisdom tooth presses against the second molar, it can slowly erode the healthy tooth's root or enamel. This constant pressure can create a trap for bacteria and food particles, increasing the risk of deep cavities, gum infections, and even bone loss between the two teeth. If left undetected, the healthy molar — which is critical for chewing — might end up needing a root canal, a large filling, or even extraction.

Since the process is often painless at first, many patients don’t realize there’s a problem until it’s too late. That’s why dentists recommend regular X-rays during the late teens or early twenties, when wisdom teeth typically start to erupt. Catching the problem early means the difference between a simple extraction and permanent damage to your healthy teeth.

If you still have your wisdom teeth and haven’t had them evaluated recently, it’s worth asking your dentist for an updated scan. Early action can save your smile from long-term complications.

We usually don’t leave an extracted tooth socket unattended. We preserve it. Advantages of socket preservation:

Long standing dental caries, trauma, inadequate filling or root canal can lead to infection and cystic swelling under the tooth. In such situation extracting the tooth is the most common solution. However with the intent of salvaging such damaged tooth some procedures can be undertaken, namely Apisectomy or Apicoectomy (Root-end surgery) with a revised root canal. The outcome is quite remarkable with the use of advanced root canal methods and bone fillers (PRF infused graft).

“Malocclusion should be and can be corrected from a very early age”

In my everyday dental practice I come across a lot of patients and specially their parents who come at a later stage with their children with some severe form of malalignment. When asked why didn’t you take treatment earlier the common answer would be it has been advised by the doctor to come later when all permanent teeth erupts.

My take on that would be not every irregularities of teeth and jaw bones need braces to start with. There are many cases which needs early intervention like space maintainers, habit breaking appliances, jaw growth modifiers etc. If we do not consider preventive and interceptive procedures it leads to severe and irreversible type of skeletal maloccclusion which becomes very difficult to treat in adult dentition.

Here is a case I want to discuss. Patient’s age is 13 years. She has one lateral incisor missing. Patient has a history of thumb sucking that persists till now. As early intervention was not taken into account it developed into severe form of malocclusion as:
- 👉Narrowing of upper arch
- 👉Bilateral cross bite
- 👉Increased lower facial height
- 👉Asymmetry in arch
- 👉Canting of occlusal plane
- 👉Anterior & lateral open bite

My recommendations to avoid these types of phenomena would be:

⭐️Look after your child’s oral hygiene by demonstrating proper brushing methods with them.
⭐️go for a regular dental check up (every 6 months)
⭐️if you’re concerned about any specific irregularities of teeth consult with a specialist.
