Case: Baimaxillary proclination.
characterized by protrusive and proclined upper and lower incisors and an increased procumbency of the lips.
Bimaxillary protrusion is a commonly seen deformity in Asian populations. This condition is usually combined with lip incompetence, gummy smile, mentalis strain, and anterior open bite.
Types
The four profile types were classified as S (straight) BM1, (bimax one), BM2 (bimax two) and BM3 (bimax three). The S profile was considered a straight or white facial profile, and the BM3 was an extreme example of bimaxillary protrusion.
Causes
The etiology of bimaxillary protrusion is multifactorial and consists of a genetic component as well as environmental factors, such as mouth breathing, tongue and lip habits, and tongue volume.
Treatment
The simplest procedure for minor cases of bimaxillary protrusion would consist of four first premolar extractions and performing anterior segmental osteotomies of the mandible and maxilla through the extraction sites. The goal would be to setback the segments as well as reduce the labial flaring of the incisors.
Mild to severe protrusion.
The retraction of both upper and lower dental arches using orthodontic intra- and extra-alveolar miniscrews, associated with self-ligating brackets, is an excellent tool to correct mild to severe bimaxillary protrusion, thus reducing the need of premolar extraction and simplifying the orthodontic management.
