Why alignment came before the crowns.

The front teeth were discoloured and crowded. Treating the discolouration alone would have left the crowding, and preparing teeth that were about to move makes little sense, so Invisalign came first.

Once the teeth were in position, the two front teeth were prepared conservatively, removing damaged enamel while keeping as much healthy tooth structure as possible. Enamel does not grow back, so every bit retained is worth having.

The crowns were then designed to match the shape and shade of the surrounding teeth, because a crown that looks obviously like a crown has missed the point.

Temporary crowns are fitted while the permanent ones are being made, usually over two to three weeks. They are not just placeholders. They protect the prepared teeth, keep things functioning normally, and give a chance to check shape and proportion before anything is made permanent.

A note if you are in the provisional stage: go easy on hard foods and staining drinks, and come back in if one needs adjusting. Quick fixes are quick.

A check-up and clean is the appointment everyone pushes back.

Nothing hurts, nothing is obviously wrong, so it drifts to next month. Then the month after.

Here is what you are actually putting off.

Your dentist looks over everything. Teeth, gums, bite, soft tissues. Not just hunting for holes, but watching for the early changes that are easier to deal with now than in two years. Most of what gets found at this stage is small, and small is the point.

Then the hygienist deals with what your toothbrush cannot reach. Plaque hardens into tartar and brushing stops touching it, so it comes off with instruments rather than enthusiasm. Then a polish, and you leave feeling smug about your teeth for roughly four days.

Last part, and the part that matters most. You get told what was found and what it means, in plain language, so you walk out with a clear picture rather than a vague sense that you should probably floss more.

Sensitivity is often the first thing you notice when a filling is starting to fail.

This case had been monitored at routine check-ups for three years. Nothing needed doing in that time, which is exactly what monitoring is for. When the patient mentioned recent sensitivity, an X-ray showed the old restoration was breaking down underneath, with decay developing at the margin.

The treatment was routine. Local anaesthetic, removal of the old material and the decay beneath it, then a new filling placed. Appointments of this kind often run around half an hour, though it depends on the tooth and what is found once the old filling comes out.

Fillings do not last forever. They spend years in a wet environment being chewed on, and eventually most need replacing. That is not a failure of the work, it is simply the lifespan of a restoration.

The reason this one stayed straightforward is that it was being watched. Picked up later, decay reaching the nerve leads to considerably more involved treatment.

This is what you walk into.

Not a row of chairs against a wall and a television nobody is watching. Proper seating with space around it, natural light through the front doors, and a coffee machine that is there to be used rather than looked at.

The waiting area was planned as carefully as the treatment rooms, because for a lot of people the hardest part of a dental appointment happens before it starts. Sitting somewhere comfortable, with somewhere to put your coffee and enough room not to be pressed up against a stranger, makes those few minutes easier than they might otherwise be.

Do full-arch implants feel like natural teeth?

Not identical. Close, though.

Dr Fong puts it this way. If natural teeth sit at one end of the scale and no teeth at all sit at the other, implants land much nearer the natural end without quite reaching it.

The difference comes down to how teeth attach. Natural teeth sit in the jaw with a ligament between root and bone, which gives a degree of feedback when you bite. Implants integrate directly with the bone, so that particular sensation is not reproduced. What you get instead is a fixed set of teeth that does not shift, does not come out, and handles a far wider range of food than a removable denture.

For someone coming from failing teeth or a loose lower denture, that is a considerable change. For someone with healthy natural teeth, it would be a step down, which is why implants replace what is missing rather than what is working.
