LOWER INCISOR RETREATMENT
This 44 year old female patient presented with pain and recurrent swelling localised to the 31 buccal region. She reported having endodontic treatment performed on tooth 31 about 30-years ago following a car accident.
Clinical assessment revealed a draining sinus in the 31 buccal alveolar mucosa. Radiographic examination revealed a large apical radiolucency. In situations like this it’s important to double check the sensibility testing because there’s a high chance of a misdiagnosis due to the extension of the radiolucency to the adjacent teeth. In this case, both of the adjacent teeth responded normally to cold testing, revealing the lesion was exclusively related to tooth 31.
Assessment showed the root filling was sub-optimal with an untreated lingual canal.
Treatment was completed over 2 appointments with an inter-appointment calcium hydroxide dressing. The draining sinus had resolved at the time of the obturation appointment. The tooth was obturated with AH-plus sealer and warm vertical compaction.
A 12-month review revealed good healing.
