New smile treatment prompted by a failing front tooth.

This was a complex treatment due to the severe deep and uneven bite. The patient was not interested in orthodontics. Treatment involved extraction of a front tooth, early implant placement and bone grafting (about 6 weeks after extraction) and simultaneous crown lengthening surgery. Prior to the CLS the gum of the lateral incisor was so low that there was no tooth visible during smiling, it looked like the lateral incisor was missing.

Then we got stuck in a temporary phase where there was insufficient space for the restorative material due to the severe deep bite (the lower anterior teeth biting onto the upper palate gum). We moved to a full mouth approach and increased the bite with a combination of onlays and crowns.

Now we're smiling.

Ceramic work front teeth and right molars . Thank you for your amazing work! Surgery, composite veneer on the front left lateral incisor, lower anteriors and ceramic-composite hybrid onlays by me and the Cerec.

After photos are 3 months after ceramic inserts.

Amazing patient, amazing lab support. I was thinking emax initially but the dark stumps and conservative preps meant Zirconia was a better option. Thank you for your amazing work.

I used for the initial smile design.

'A million times better than before'

It's nice to hear this feedback from patients πŸ™

This gent had been to Thailand for a quote, thanks to .ellie.knight.dentist for suggesting he have a consult with me. Part of the Thai plan was root canals for the lower incisors. Luckily we were able to avoid that.

Many helpers for this job, and over 45 hours in the chair with me.

Thanks to my super staff for the scheduling (and rescheduling!). Thanks to for the retreatments 12-22, and molars to come. Thanks to and for the wonderful printed temps (photos 3 and 4). Saved me hours of composite transfer tedium! Thanks to team for wonderful lab support. And thanks to most excellent teacher on importance of occlusal plane... hopefully we did ok.

And thanks to the patient for choosing to keep his teeth. 🦷

Smile Renovation πŸ› οΈ thanks for the analogy

It's nice to save what's there.

This lovely man was noticing his front teeth chipping. Between our consult and when he started treatment the front right tooth chipped a lot more. Luckily it was still salvageable.

He noticed that his back teeth weren't really touching anymore (see the darker blue marks on the front teeth in the last pic). This meant all the functioning was on the 'children' as says. The touching on the front teeth should be light. The back teeth, or the 'men' should be doing the chewing work. The lower front teeth are ceramic, aggressively wearing down the front top teeth. The back lower teeth were very worn and eroded.

To fix the front teeth we needed to make space for them first by building up the back teeth.

Looking at the whole picture... not just what you see in the photos. This addresses the cause of the problem and gives us a longer lasting solution.

Technical Stuff:
Opening the VD with bonded cerasmart under rubber dam. Anterior teeth built freehand in composite. I did the wax up (with real wax lol), a try in with luxatemp, and then used a palatal key to do the 13-23 in composite. Upgrade to porcelain possible.

For the dentists that do this type of treatment... it's hours and hours of work. Why are we drawn to it? Hours of preparation, and hours of clinical time. What is enjoyable? The outcome? Of course it's beautiful to help people. But why else? The planning? The doing? I think for me the doing and the flow state is the most enjoyable. And the learning. Always trying to be better. This case for sure photos could be better 🫢

- evened lower occlusal plane with composite on the lower premolars
- gum lift for some upper teeth
- ceramics on all the uppers
- help from our specialist friends for some gum treatment and root canal
- no orthodontic treatment
- night guard

When Two Will Do 🦷 🦷

The technical stuff:
Lovely lady had noticed her front teeth were getting shorter. We could verify with photos she had of herself from a few years ago. As clinicians our first question shouldn't be 'how can I fix it?', but 'why is it broken?' What is happening to cause the defect? Will my repair also succumb?

In this scenario the culprit was pumpkin seeds, many chewed over many years with no apparent problems. And then what seemed to the patient like a rapid increase in 'shortening'. Most likely the rapid rate of wear coincided when the enamel was breached and dentine exposed.

In this case our patient chose ceramic over composite due to the superior physical properties. She enjoys red wine and didn't like the idea of staining the composite.

A chair side mock up revealed these would be mostly additive in overall design, meaning conservative perps that were mostly in enamel. Happy teef!

Congratulations to this handsome guy on his new smile!

He had been suffering with sharp teeth cutting his lips and tongue and never wanting to smile because he looked like he was scowling. Sharpness resolved. Scowls now delivered at his discretion!

The technical stuff:
This was a staged FMR guided by Francesca Vailati's 3 Step Technique of Additive Prosthodontics.

Mock Up:
- testing VD tolerance: a simple wax up and mock up I did to analyze if the required restorative space for the anterior ceramic thickness would be tolerated
- AP relationship at an opened VD: in this case we were able to improve the edge-edge bite to minimal OJ and OB by opening the vertical
- Aesthetic preview: showing the patient what 'regular' size teeth look like after years of broken down teeth (and AP relationship) can feel unnatural, huge (and bucky) for the patient
- Space Maintenance: in what may be his most important but underrated job, the mock up keeps the space! These jobs can take months, (in this case we waited 3 months for the surgical crown lengthening to stabilize). In that time the space created anteriorly by adding to the posterior teeth can disappear, especially if the anterior teeth were living life overloaded. The pt was able to wear this removable mock up for several hours a day to help minimize space loss. Alternatively we could have done in fixed composite.

Posterior Build Ups and Surgical Crown Lengthening:
- Molars Emax, lower premolars prepless Cerasmart bonded with heated composite.
- SCL to even gingival margins and improve abutment height and retention for 12 many thanks demystifying SCL

Lower Anterior Composites
- no enamel harmed. Embrasures and shade could have been better

Upper Ceramic
- Preservation of bulk of palatal enamel from our planned VD increase, pal margin only
- Custom shade, Emax everywhere and Zr cantilever (24)25 bridge. Thank you

Over 30 hours in the chair this treatment is no joke! Commendation to patients who choose this treatment! Long live teeth!

Some interesting treatment with this lovely patient. A lot of wear. The top left lateral incisor was having a hard time staying where it had been placed with lingual braces 8 years ago; it was in conflict with the lowers during function. We created some freedom with opening the bite with composite, cerasmart and emax. Restoring function and aesthetics. Patient's amazing hygiene going a long way to improving the overall situation, especially the lower anterior teeth.

This case was good, and the patient was great πŸ€—

Desire to improve the smile. We started with digital planning, crown lengthening surgery, endodontic assessment and retreatment , Zoom!in chair whitening, a resin filling and 4 zirconia crowns. After an early fracture of the heavily root filled central incisor while I was at a conference in Singapore 😫 The whole Dentista team totally stepped up πŸ™ Dr Saul Todres helped amazingly clinically πŸ™ upon return I placed a direct fibre post, created some serious ferrule, and printed some beautiful splinted resin temps for us to stabilise and trial for a few months, then we proceeded to the final ceramics. Thanks to everyone who helped out with the case
