When a heavily restored tooth can’t be saved

This upper right canine had a large existing restoration with significant loss of natural tooth structure.Bone levels were low and the tooth was loose.

The X-ray and clinical examination indicated a poor prognosis, and extraction was chosen following discussion of the treatment options.

The tooth was removed under local anaesthetic.

🔎 After extraction, inflamed soft tissue was visible attached around the tip of the root, consistent with inflammatory tissue associated with infection in the area.

Whenever possible, we aim to preserve natural teeth. However, when a tooth has a poor or unpredictable prognosis, extraction may sometimes be the most appropriate treatment.

🦷 Wisdom Tooth Extraction

Deep decay had reached the nerve of this upper left wisdom tooth, causing irreversible inflammation and pain.

Following clinical assessment and discussion of the treatment options, the tooth was extracted under local anesthesia in the dental chair.

A “broken tooth” can sometimes be more than just a chip.

The OPG and intraoral examination revealed deep caries affecting both lower wisdom teeth:

🦷 48: A large cavity was clearly visible intraorally, with the OPG showing decay extending close to the nerve. The tooth was painful. The patient declined root canal treatment and chose extraction.

🦷 38: Deep caries was also present, but pulp sensibility testing was normal. The patient chose to save the tooth with a filling and understood the risk of pulp exposure and possible future treatment.

Don’t wait for pain—early assessment may provide more treatment options.

Cracked Tooth with a Deep Filling

The patient presented with discomfort from a heavily restored tooth. After removing the previous filling, underlying decay and cracks within the remaining tooth structure were identified.

The tooth was carefully cleaned and managed conservatively with:

• A protective Life liner
• Riva glass-ionomer base
• A direct composite Onlay to restore and reinforce the tooth

At review, the patient reported relief from their symptoms.

Cracked teeth can have an unpredictable prognosis and require ongoing monitoring. Further treatment, such as cuspal coverage or root canal treatment, may become necessary if symptoms return or the crack progresses.
