Inspired by a recent story by and dealing with complex defects left by failing implants.

A young male in his 30s presented for a second opinion regarding management of the UL1 implant. History of trauma to the area and the UR1 is necrotic with apical periodontitis. Patient also had generalized 4-7mm pockets and Stage III Grade C periodontitis. A combination of endodontic treatment, bleaching, nonsurgical and surgical periodontal tx, hard and soft tissue grafting. Thanks for the excellent restorative by .gidonfixler.

Each site (LR1 and LR4) having their own requirements for periodontal therapy and tissue management.

LR1 treated with an FGG
LR4 had residual probing after a nonsurgical phase of 6-7mm labially that was treated with sulcular flap approach with debridement, amelogenins (Emdogain) and a simultaneous CTG.

6 months we have improved probing depths, recession coverage, attached tissue and keratinised tissue width.
