Step 1:
💬 Schedule a consultation appointment before booking your Philips Zoom! In-Chair Whitening treatment.
Step 2:
📲 Once your treatment is complete, scan the QR code on our counter card before you leave.
You’ll complete a quick online form to claim your FREE Sonicare 2000 Electric Toothbrush.
To validate your claim, you’ll need to:
✅ Upload a clear photo of the counter card displayed in our practice (snap this before you head out!)
✅ Include your receipt/invoice number in the claim form
⚠️ Claims missing this photo or valid info will be rejected, so please make sure it’s done before you go!
Step 3:
📦 Philips will ship your Sonicare 2000 directly to you within 3–4 weeks.
🗓️ Important Dates:
• Treatment period: 1st November 2025 – 31st January 2026 (inclusive)
• Claim submission deadline: 28th February 2026 — late submissions won’t be accepted.

⚠️ Terms & Conditions:
Offer valid only for eligible patients who complete a full Philips Zoom! In-Chair Whitening treatment at Logan Village Dental between 1 November 2025 and 31 January 2026.
Patients must submit a valid claim via the official Philips claim form, including a clear photo of the in-practice counter card and treatment receipt/invoice number, before 28 February 2026.
Claims missing required information or submitted after this date will not be accepted.
