Your health and safety are our highest priorities.
To help prevent medical emergencies and avoid potentially dangerous drug interactions, all patients are required to keep their Medical History up to date.
Under the requirements of the Dental Board of Australia, dental practitioners must check and record relevant changes to a patient’s medical status before commencing treatment. This means you may still be asked to update your Medical History even if there have been no changes to your health.
You can update your Medical History annually by:
• Using the secure online portal when you make an appointment, or
• Completing an updated form when you check in for your appointment.
Our team will also ask about any changes to your health during your regular visits.
Keeping your Medical History current helps us provide your dental care as safely, responsibly and compassionately as possible.
Regards,
Dr Michael Cooke and Dr Cathy Findlay
