No extractions, no surgery. 14 months of braces + expanders resulted in this beautiful smile. This patient was told by another clinician that 4 adult teeth had to be removed. We completed this treatment with zero extractions.

Extraction-based orthodontics is a well-established, effective approach, and for plenty of people it's the right one. But it isn't the only starting point, and it's worth understanding the alternatives before deciding.

Here, the plan was 14 months of Guided Growth, gently guiding the dental arches (the curved ridges of bone that hold the teeth) to develop with more width, so the teeth had somewhere to settle rather than needing space made for them. A lot of adults are told it's braces or surgery. This was done with no Surgery and no extractions.

Look at the upper arch in slide four. The before photo shows a narrow, V-shaped arch (the curved ridge of bone that holds the upper teeth), with the back teeth pulled in and the front teeth crowded and pushed forward. The after shows a wider, rounder arch with everything sitting where it has room to be.

This was treated without jaw surgery. The approach was to develop width in the upper arch first, then use braces to settle the teeth into that new space. It takes patience, and it takes an assessment that looks at the whole system rather than just the front teeth.

A narrow upper arch means less room for the tongue, and where the tongue sits can influence how easily air moves through the nose, particularly during sleep. Over 9 months with expanders and 11 months in braces, the upper arch (the curved ridge of bone that holds the teeth) was guided to develop with more width, so every tooth had somewhere to settle. No teeth were removed to create that space.

Reduced Pain Tolerance: Sleep-deprived individuals often have a lower pain tolerance. This means that even mild to moderate pain can become unbearable, further impacting your ability to cope with chronic pain.

Inflammatory Response: Sleep deprivation can trigger an increase in inflammatory markers in the body. Chronic pain conditions are often associated with inflammation, and poor sleep can worsen this inflammation, leading to increased pain levels.

Previously, at 7 years old, recomended that adult teeth would be removed as a teenager.

Expansion orthodontics allowed this patient to develop a broad, healthy smile without the need for any extractions. After the expansion was finished, we completed the job with alignment using braces.
