I use to get a little worried I couldn’t find the impacted tooth when I first started learning about wisdom teeth surgery. There are some good clues- Obviously the imaging is helpful, especially CBCT’s (cone beam cat scans), but you still have to do the surgery and manage the blood, saliva, and visibility, etc.

Painting the bone really refers to the bur as it selectively wants to cut the softer bone over enamel. So lightly going over it until you feel and hear the chattering of the enamel is super helpful.

Mesially inclined, impacted wisdom tooth extraction after a buccal trough and purchase point. I was able to elevate the tooth up and placed another purchase point lower down the root. The tooth had an extra root as well.

Mesioangular Wisdom Tooth Extraction with Distal Crown Removal and a Purchase Point

Some questions I get- “what bur is that?” and another one is “what order do you remove wisdom teeth?”

I switched to a 1703 bur from a 703 bur 2 years ago. The 1703 has a rounded tip and I like it better for finding complete bony impactions and cuts smoother with less chatter.

If I am doing an IV sedation I remove the two lower wisdom teeth first then the two uppers. The uppers are usually faster extractions but actually more stimulating for the patient (that pressure). So I do those at the end so the sedation stays smoother. My logic works for me lol.

A shallow buccal trough was all that was needed for these lowers.

📌If you feel tissue still attached when pulling the mobile tooth out don’t keep pulling and hoping for it to snap off. That usually ends up causing a laceration. Find the attachment and carefully snip it.
