I used to remove four wisdom teeth in this order: bottom right, upper right, bottom left, upper left.

About 5 years ago I changed to this:
Bottom teeth then top teeth

I like this for a few reasons- The upper teeth are generally quicker to remove although when they are tough they can be really tough. The uppers are also very stimulating for the patient. The pressure is an unpleasant stimulus so if performing IV sedation, just that pressure in the middle of the case can disrupt an otherwise uneventful sedation. So I leave them for the end.

This logic seems to work for me but who knows if it makes a huge difference.

Lower lip mucoceles are fairly common. Minor trauma to the lip can cause the minor salivary ducts to tear or twist and create a back flow of saliva. A dome shaped swelling on the lip can follow. Sometimes the swelling has a characteristic “gets bigger and smaller” pattern or a bluish hue to it.

Treatment is often local incision and including the minor salivary gland is helpful to minimize recurrence.

It took twice as long to get from 40 to 50K but the algorithm is what it is. I have received thousands of messages since on jumped on insta in 2020 and try my best to respond to as many as I can. If I helped you out at all from my content or from messages feel free to share your story, give a repost and continue sharing my stuff.

People that post surgery content are pretty much shadowbanned so the algorithm doesn’t share our stuff and often actually suppresses it. So the only way we can grow is by those getting value from it to take a couple seconds to repost and share it. So just remember that next time you see something out there that inspires or helps you and give that creator a repost and share. I am off my soapbox now lol.

I use to get a little worried I couldn’t find the impacted tooth when I first started learning about wisdom teeth surgery. There are some good clues- Obviously the imaging is helpful, especially CBCT’s (cone beam cat scans), but you still have to do the surgery and manage the blood, saliva, and visibility, etc.

Painting the bone really refers to the bur as it selectively wants to cut the softer bone over enamel. So lightly going over it until you feel and hear the chattering of the enamel is super helpful.

Mesially inclined, impacted wisdom tooth extraction after a buccal trough and purchase point. I was able to elevate the tooth up and placed another purchase point lower down the root. The tooth had an extra root as well.
