Zygos, ZIP-flaps, subs, and fibulas — the title alone is going to spark hallway debate. The real question underneath it: are obturators archaic?
Dr. Jayanetti (Chief of Maxillofacial Prosthetics, UCLA) walks through UCLA's treatment algorithm across 50+ patients — zygomatic implants as first-line for larger maxillectomy defects, subperiosteal frameworks when endosseous placement isn't possible, ZIP-flaps as an alternative to fibula-based "jaw-in-a-day" reconstruction. His answer: obturators aren't obsolete — they're one tool in an expanding, patient-specific algorithm.

Dr. Joseph Meleca is an assistant professor at University of Chicago. Director of the Facial Plastic and Reconstructive surgery. Will present his work on the anterolateral thigh fasciocutaneous rescue free flap for mandibular osteoradionecrosis. Osteoradionecrosis of the mandible is a dreaded complication and sequela of radiation, specifically for head and neck cancer. Will describe a novel technique that has been around for nearly 10 years that works to provide vascularity to the tissue bed when conservative therapies have failed or when a patient is not yet a candidate for more extensive surgery like the fibular free flap. Some of the objectives are to learn indications, tips, and outcomes.
