🟣 In our latest video, Dr. Alikhani continues the anchorage discussion in Part 3 of the series.

This session focuses on another strategy to reduce movement of the anchor unit: differential forces. With a brief review of the biology of tooth movement and simple illustrations, Dr. Alikhani explains how elastics can optimize the force applied to the Target unit while minimizing the forces on the Anchor unit.

While widely used to reduce anchorage loss, this approach must be applied carefully to avoid unintended side effects.

🟣 In this video, Dr. Alikhani reviews a facial trauma case complicated by delayed orthodontic follow-up after hospitalization, which resulted in tooth ankylosis and an open bite.

Against the common belief that ankylosed teeth cannot be treated, he presents a CTOR Ortho approach that includes subluxation of the ankylosed tooth and forced eruption using precise mechanotherapy—achieving stable anchorage, open bite closure, and an ideal occlusion.

🟣 In this new video, Dr. Alikhani reviews the craniofacial and functional implications of Nemaline rod myopathy (NM), a rare congenital myopathy. Altered muscle function in NM can lead to significant challenges — including skeletal open bite tendencies, deep palate with thick mucosa, flaccid soft palate without proper nasal seal, swallowing difficulties, tongue-thrust patterns, and reduced condylar and coronoid development.

Understanding these neuromuscular effects is essential for planning orthopedic and orthodontic care, highlighting the need for interdisciplinary collaboration, tailored treatment strategies, and adequate neuromuscular adaptation time.

🟣 A new Biology of Orthodontics video is now available on CTOR’s YouTube channel.

Dr. Mani Alikhani presents findings from a clinical trial examining how age affects the biological response to orthodontic forces. The study shows that beyond a certain threshold, increasing force does not enhance tooth movement—both adolescents and adults reach a saturation point, though adults reach it sooner.

These results highlight the need to individualize force levels in orthodontic treatment based on biological response rather than magnitude alone.
