preventative and restorative dental care, oral surgery, mental & behavioral health services for children and adults, Psychiatry, Health Insurance Enrollment Assistance, substance use disorder treatment, and specialty services including optometry eye care, optical shops, nutrition consultations, podiatry, TMS Clinic, Spravato Clinic and Outpatient Day Treatment. CHC has five sites within Fitchburg, Gardner and Leominster with decades of experience as a Federally Qualified Health Center (FQHC), serving 36 communities in North Central Massachusetts. All CHC health centers are located on the Montachusett Regional Transit Authority bus line. Community Health Connections was established in 2002 and is a Federally Qualified Health Center (FQHC). Community Health Connections (CHC) is a mainstay for compassionate care, regardless of income or insurance status. Our mission is to provide high quality, affordable health care in our community. Our community of patients includes insured, underinsured, uninsured, low income, publicly housed, and homeless populations.
