As endodontists, we are always judged on the results of that final post-op X-ray but that’s never the complete story. We usually get really difficult patients; highly anxious, “feel” everything, can’t open very wide, oddly angled teeth, gag often, need to sit up frequently, second molar with apices on the IAN and on their phone while we are trying to do the perfect root canal!! So that “final result” that we post is the pride we feel just getting to the end and not always the pretty looking root canal. When we accomplish both: that’s the WIN!! 🏆

Are you afraid of removing Thermafills? Well, here is one that caught me off guard! The post op is pic 1 to keep the home page legit! 🙃 Now scroll 👉🏽 to see some of what we were dealing with here! When I looked at the PreOp or CBCT I thought straight forward ReTx. When I got in there; surprised by a post and Thermafills!! Ever happen to you?! Here is my method: I used an ultrasonic to remove the post, some headstroms, and a bit of prying to remove! All out in 15 min!! If you want more detail; send me a message! Let’s walk through an Apicoectomy of tooth #20!
First, I get it..option of extraction vs doing nothing vs endo are always on the table!! Especially when you’re talking about an Apico near the mental nerve!! This patient understood the consequences and potential damage with numbness was addressed! They wanted to try and didn’t want to loose another tooth!!
CBCT revealed a previously treated RCT with large PARL. Potential cracked/fractured root was discussed..hence extraction conversation! Root was 18mm with 2 mm space to the mental nerve.
Planned flap to extend from tooth 19-21 to gain access and visualization. CBCT measurements help and identified 2 locations of entry. After verification with PA X-rays shown, identified, located and visual identification of mental nerve was made. Troughed a 4 mm horizontal “line” 1 mm above mental nerve to secure retractor.
Proceeded to make osteotomy to identify lesion. Upon removal, identified tooth apex for resection. When lesion and resection was visible and clear, retro-prep made, retro-fill completed and bone graft was placed.
Visual inspection of mental nerve was made one last time prior to suturing to ensure it was intact without damage! Sutures placed and final x-ray taken. Reviewed outcome with patient with great optimism! Hope you get something out of this one! Pat on the back for this tough surgery!

As an endodontist; we don’t get easy cases and I don’t give up on teeth easily!! I’m partially proud of this one, not because it looks great but mostly because we accomplished our goal to save this tooth!! Super tough to go half way into the mid-root area to find the 2 canals, negotiate them, and clean and shape up to a 20.04! Trust me when I say; I really tried to get them to a 25 or higher and was becoming cross eyed!! After we completed the Rct/core; we proceeded to surgically repair the resorption that was subgingival and into the root surface! 😅 I did all of this all in 1 visit. Not sure a second visit would change anything and was worried about not seeing the 2 canals again or damaging the canal looking twice!!
Definitely worked hard on this case and just wanted to share and keep it on my page to remember it! 💪

Perforation with External Resorption
The patient stated that he had some trauma when he was younger and had this root canal and post placed over 10 years ago. He said he’s always felt like something felt odd but said the dentists through the years didn’t really say anything was wrong - until; he developed some swelling in the facial side of the tooth. SCROLL 👉🏼 to see the where we started. CBCT revealed perforation due to misaligned post placement. Surgical repair with 6 month recall at the end. No recession, no probing past 1 mm, and excellent healing!
