A little show and tell! Remember the classic lit study by: Senia, 1971?! The Tissue Dissolution Study that evaluated how effectively NaOCl dissolved pulp tissue compared to normal saline. Then later the 1975 the “1 minute” decontamination of gutta percha. So, my assistant wanted to understand why we rinse with what we rinse with. We took a nerve that was solid out of a palatal root, placed it into a dish with 6% NaOCl for 1 minute, and agitated with endo activator. By the time we agitated, the tissue was already broken down. Here is a quick “time lapse” of our chair side experiment! 🔬

Another successful recertification for CPR and OSHA requirements! Always a good time getting together with Dr. Alexander Smith Dr Smith and his awesome team members! HP was great and makes it real!! We also had a moment to celebrate Dr. Smith’s and his wife’s Birthday 🎂 weekend too! We appreciate their loyalty, collab with patients and ongoing friendship! 🤩 We’ll look forward to the next get together!

🦷 So, I see a ton of really great Endo on social media and a lot of those inspire me to always try harder, have more patience, and do something else in my protocol. I have to admit; it makes me feel a little insecure at times. I wish all my cases looked like yours and I do strive for pretty looking endo to post and share. Unfortunately, there may be only 1 case out of 10 in a day that I want to share here.
Here is a case that isn’t pretty but took a ton of work. Couple other guys said to extract this one and don’t waste the patients time or money. I think I was the 3rd opinion (sadly, not the first) but I’ll take it. Again, another hit to my tiny ego! As the last guy 🤣 we decided; let’s ReTx it and see where we go with a questionable prognosis and a post to removal and 2 other Endo wanting to extract it.
I don’t back down easily and I’ll admit; the tough cases always make me better! I teach a lot but I still learn a lot!! The “feel good” comes not at the finish post op; but when this patient came back in 6 months with great signs healing!! ❤️‍🩹 Not sure who was more excited; me or the patient! 😌 Hope you enjoy my humble admission and the great feeling of saving one!!

When the body heals from a procedure that we did; it’s a gratifying feeling!
I’m sure we all have those cases that we are skeptical of the outcome. Sometimes we try and other times we never know, because the tooth is taken out instead. When it works, we still may never know it healed, because as an endodontist we don’t recall every patient.
I recall those tough ones! It gives me an idea of what’s possible, what works, what kind of outcome are we getting from my own procedures, skills, and protocols. This one was one of those cases! Another happy patient for sure! 😊

Here are two of today’s that should just be “easy” but they weren’t. What you can’t see here is the rotation of the teeth because when they prepped for the crowns; they corrected that rotation. Nothing wrong with that, but if you’re doing root canals on upper premolars, through a crown, be very careful with your access. You can find yourself sitting outside of that tooth or roots very quickly. Also, love the apical splits on this one too! 😊

As endodontists, we are always judged on the results of that final post-op X-ray but that’s never the complete story. We usually get really difficult patients; highly anxious, “feel” everything, can’t open very wide, oddly angled teeth, gag often, need to sit up frequently, second molar with apices on the IAN and on their phone while we are trying to do the perfect root canal!! So that “final result” that we post is the pride we feel just getting to the end and not always the pretty looking root canal. When we accomplish both: that’s the WIN!! 🏆

Are you afraid of removing Thermafills? Well, here is one that caught me off guard! The post op is pic 1 to keep the home page legit! 🙃 Now scroll 👉🏽 to see some of what we were dealing with here! When I looked at the PreOp or CBCT I thought straight forward ReTx. When I got in there; surprised by a post and Thermafills!! Ever happen to you?! Here is my method: I used an ultrasonic to remove the post, some headstroms, and a bit of prying to remove! All out in 15 min!! If you want more detail; send me a message!
