The patient presented with a history of mobility and root exposure of the maxillary right first molar (Grade III) and second molar (Grade II), with the first molar showing root exposure for the past one year. Clinical and radiographic findings were suggestive of chronic infection and suspected osteomyelitis.

Under local anesthesia, extraction of the maxillary right first and second molars was performed. Following extraction, thorough debridement of the socket and surrounding infected bone was carried out to manage the suspected osteomyelitis.

Due to the large oroantral communication and soft tissue deficiency, buccal fat pad advancement flap was performed on the right side to achieve tension-free closure of the defect. The flap was stabilized and closure was achieved using resorbable 3/0 Vicryl sutures in interrupted fashion.

The patient was advised appropriate post-operative care, medications, and follow-up to monitor healing and rule out further infection.

The patient presented with an communication (OAC) following maxillary molar extraction performed by a local dentist. Clinical and radiographic evaluation confirmed the presence of a persistent communication between the oral cavity and the maxillary sinus.

After obtaining informed consent, the site was explored and thoroughly debrided. Platelet-rich fibrin (PRF) was prepared and placed into the defect to promote soft tissue healing and act as a biological scaffold.

Closure of the oroantral communication was achieved using a pedicled buccal fat pad (BFP) advancement flap. The BFP was carefully mobilized, positioned over the defect, and sutured in place to ensure tension-free closure. The overlying mucosa was then approximated with resorbable sutures.

Postoperative instructions were given, and the patient was advised to avoid nose blowing, sneezing with a closed mouth, and use of straws. Follow-up was scheduled to monitor healing and assess for any signs of sinus involvement or flap dehiscence.
