Third molars, or wisdom teeth, typically erupt between the ages of 17 and 25, and in many cases, they don't have enough space to emerge properly. This can lead to irreversible damage such as resorption of adjacent roots, cysts, infections, and jaw fractures.

Patient came with severely damaged upper incisors that had already undergone multiple treatments. A 16-year-old patient presented with a pain in left mandibular first molar requiring root canal therapy. During access cavity preparation and canal exploration, four distinct canals were identified — two in the mesial root (mesiobuccal and mesiolingual) and two in the distal root (distobuccal and distolingual).
Although the mandibular first molar typically presents with three canals, the occurrence of an additional distal canal is a common anatomical variation.Use of magnification, adequate illumination, and careful exploration of the pulpal floor greatly enhance detection of such additional canals. This case highlights the importance of respecting root canal anatomy because every canal counts!
