Diagnosis : SIP
Canals: 4 (MB,ML,DB,DL)
Irrigation: 5% NaOcl and 17% EDTA with closed ended side vented irrigation needle
WL: PA plus Apex locator
Obturation : CLC
Final restoration: fiber post and core

Case started in teaching hospital , referred to SMDC-OIC
DL was completely blocked and other canals had ledges ..

DL (Radix root which was highly curved) root had been opened , prepared and obturated by
Master Dr Shoaib Alam 🙏

A patient presented with a complain of pain in LR6 ttp +ve , previously she had been treated 10 years back with Root canal treatment through thermafil obturation technique done in US.. followed by filling (history is filling was dislodged and tooth was open)
Leakage was the cause of infection in previously treated tooth..
Although treatment was excellent but here is the striking point , core buildup with marginal seal is what makes your treatment long term successful..

As someone has rightly said
"SEAL IS THE DEAL"
I did retreatment and find thermafil carriers in all four canals..
WL: periapical X-ray
Thermafil carriers removed by gates Glidden
Obturation: CLC , sealapex
Irrigation : 5%NaOcl ,17% EDTA
File system : rotary gold protapers
Canal system: 4 canals (1:1)
Post and core: 2 fiber posts in DL and MB

Patient is on follow up, hoping for the best InshAllah 🙌

C-shaped cases look easy but difficult to execute..
To understand this difficult morphology , cone shifted radiographs are important..
