This extraction was initially planned as a simple extraction; however, due to intraoperative challenges, it progressed into a surgical procedure.

A dentist opts for a surgical approach in the following situations:
• When there is failure of simple extraction techniques, such as inability to luxate or deliver the tooth
•In cases of grossly decayed or fractured crowns, where adequate grip with forceps is not possible
•When dealing with root fractures, retained roots, or dilacerated roots
•Presence of impacted or partially erupted teeth
•When there is dense surrounding bone or limited access
•In cases of ankylosed teeth
•When proximity to vital structures requires controlled bone removal and sectioning
•When excessive force during simple extraction may risk damage to adjacent teeth or bone

Such situations often require more time and may be stressful for the patient, especially when the procedure was not initially planned as surgical. Clear communication and reassurance become essential.
